Lower back and abdominal cramps
« on: October 30, 2025, 11:27:19 PM »

Hi All,

I’m 52 and my HRT regime is Evorel 50 and 100mg continuous Utrogestan. I was still having regular bleeds before starting this regime.
With the agreement of my GP I wanted to take the Utro continuously to avoid a dip in my mental health- I have had GAD and depression since my late teens.
I haven’t had a period since starting this regime but have recently had cramping in my lower back and abdomen, with a sharper pain occasionally in left lower abdomen. To be blunt I feel like crap.
Could these symptoms be my body screaming out for a period? Should I take the Utro cyclically? Perimenopause is a minefield and I’m so fed up 😭. These aches and pains aren’t helping my anxiety as I’m convinced that something sinister is going on  :'(
To clarify- I have had a few blood tests including CA125 and have had a vaginal swab, all of which were clear.

Thank you for reading ❤️.

Hi jedaka

 :welcomemm:

Sorry to hear you're suffering.

If you were still having regular periods when stating HRT and no sign of peri-menopause (ie variability in cycle length from month to month combined with some classic menopausal symptoms, and your age) then you will still likely ovulate as the utrogestan won't be sufficient to suppress  ovulation. That being the case then then you could easily have some sort of period monthly, albeit lighter due to the progesterone taken continuously.

You didn't say how many months you've been taking HRT, when you say you haven't had a period since starting HRT but those symptoms sound just like the beginnings of a period to me. In fact I am surprised that you havent had one if it's been several months?

There is no harm in taking progesterone continuously but you just maybe won't know when to expect a period, though if they are still "regular" ie ovulation, then you should get some spotting or period symptoms monthly until ovulation beings to become sporadic.

Glad your investigations have been clear! Try not to worry!
