You can have vaginal oestrogen with a thickened endometrium ! The dose of oestrogen is very low.

I just wanted to update you on what happened at my appointment yesterday. It was all good news! When I was rescanned the lining was 3.8 mm and the gynaecologist said that depending on which way the probe is angled the lining thickness can vary which obviously has an effect on the cut-off points if it's around 4mm. He didn’t think it necessary for me to have a hysteroscopy as he believes it’s the progesterone component of the HRT that is causing the bleeding. He said nothing looks suspicious on my scan so he’s gonna go on common sense. He also mentioned that too many women are being sent on cancer pathways for post menopausal bleeding on HRT when the chance of cancer on HRT is so low. He did a smear test as I have a lot of cervical pain. Getting results next week. I’m going to have a follow-up scan in five months to check lining and see how I’m doing on a different regime of progesterone taking Utrogestan orally.

He said if I am still bleeding taking it that way he will discuss the Mirena or a combi oestrogen/progestogan patch. The atrophy he thinks is made worse using the Progesterone vaginally and he will treat that when I've switched to oral Utro.

Thank you so much CLKD and Dotty for all your advice!

:thankyou: 4 the update!  Your Guy sounds like he knows what he is talking about, hang onto him ;-)

MayB keeping a mood/food/symptom diary may be of use to chart progress?

Are you on treatment for va? If you're not I would suggest you use a loading dose for 3 weeks and make your appointment for after this. If you are on treatment and still have symptoms you need to increase. Some doctors consider 5mm OK if you're on hrt so ar 4.3 I think they're worrying unnecessarily. Hrt causes fibroids to grow and bleeding isn't uncommon when you have them. Has anyone considered removing your womb as this would sort out both problems?

Thanks for the mood diary suggestion CLKD. I have to switch to oral Utrogestan which I'm being a big baby about in case of side effects. I really hope it helps me sleep. Yes he is a very kind gyn from the UCLH team.

sheila99 - I am not having a hysteroscopy now as the consultant I saw rescanned me and said the lining was 3.8mm so he didn't think it necessary. He does not want the fibroids removed either. I don't think a hysterectomy is on the cards. I have Ehlers Danlos syndrome so surgery like that could be risky for me. He will treat the VA once I've switched the route of Utrogestan. I have severe cervical pain so and I'm waiting on a smear result which I will get on Monday.... :'(
