Hi all its me again after many years on hrt changed to ultrogeston / oestrogel in feb ive be bleeding or spottong all thw time since starting it i take it vaginally as tried for a few weeks orally and it made be feel awful stomache cramps and dizziness, anyway my gp aa now said to try back orally to see if bleedimg stops so im doing so much research as i feel so good on this hrt i want to try and make it work, now i read somewhere taking it vaginally hits the spot better so im thinking do you think perhaps im having to much thats why im bleeding and if i go back ti orally as it breaks down in the body i might get less, my gp didnt say why to go back orally 2 be honest it was a rushed call off her, by the way i waa seen in gynecology in feb for polyps so everything is ok down there i hope im making aense on this as besides the bleeding i feel good and gp as tiold me if bleeding dont stop the next step is a mirena coil and im not having one by the way ive also tryed upping the ultrogeston to 200mg i just blees heavier ,.

Hi Jan I am also on oestrogel and utregestan. I posted the other week about bleeding which started when I reduced the utregestan from 200mg to 100mg. As a trial I tried the utregestan vaginally and the bleeding stopped! I did notice a minor ‘cramp’ sensation in the first few days - a bit like a very mild period pain - but that has gone now. I am on 4 pumps of oestrogel and 100mg of utregestan now and it’s suiting me well. If I take the utregestan orally I need 200mg to achieve the same effect. I’ve just started using my utrogestan vaginally on GP advice as orally wasn’t controlling bleeding. Been ok so far although it’s only been a couple of weeks. Would usually have been bleeding by now though. My GP is also pushing me for a Mirena coil if this doesn’t work and I too don’t want that. I’m using 100 mg utrogestan vaginally every night.

Without knowing your story I can't comment but bleeding on continuous HRT is often sue either to cycle breaking through or the progesterone not being sufficient to control the bleeding (compared with the oestrogen dose which causes the lining to grow), or that the lining has built up on another regime and adding/changing the progesterone might be causing it to shed unpredictably.

Occasionally also spotting on very high dose of progesterone with lower doses of oestrogen can mean the womb lining has become too thin but I think that is less common? (No stats on that though).

Unless your doc has suggested having a scan you could try taking it cyclically (vaginally) for a few cycles which will mean the bleed is predictable - and then go back to continuous and see if that helps?

Hurdity hi im 57 and been on hrt fof about  8 years, back in november i had spottinng i was on klofem then gp asked me to change to ultrogeston / gel and referred me to gynecology as she could see a cervical polylp afer examination, seen gynecology dec hax a scan lining was 17mm had a hyposcopy done and was told large polyp / fibroid she told me to stop hrt until they was removed as it will help with the bleeding and the lining was thick because of the polyp, so stopped hrt 8 weeks no bleeding at all then feb re started and had problems since, i did ask gp this week if spotting anf bleeding was because of my lining and she said not now because ive had heavy bleeds and it should be fine i asked for another scan and said no not needed, ive tried 200mg progesterone like advised and bleed heavier,

Hi Jan,
Could you take the utrogestan cyclically as Hurdity mentioned?
That way, your bleed would ( hopefully) be more predictable.

Hi nas i used ti be on it that way up till 2019 and changed to conti as i suffer with terrible period pains so dont really want to now at 57 but if i thought it would help i can suggest it to my gp, ive acually had 3 weeks free of bleeding until last week i yake mt hrt at night and my mid morning i spot not alot but every time i go for a wee theres slight spotting when wipe, i then stop late afternoon and it repeats again then following day i just cant understand where im going wrong

Yes at the moment with period pains this as happened previously and then the bleed came on,.

If bleeding doesn’t settle after six months or so of being on a new regime, then it’s probably time to think about an alternative progesterone.

Nas gp as prescribed tablets for me to stop bleeding  if it comes back heavier if that dont work she said il have to have a mirena im not having one i asked her if i could go back om kliofem and she said they wont work, i dont know what to do now i can only try what she as advised thats to up gel to 3 pumps and take ultrogeston orally thanks for the reply

What a pain Jan1234! You must be frustrated with it all.

I don't think taking it orally is going to make a difference, based on my own experience. I find that using 100mg vaginally is enough with 2 pumps oestrogel. I had more breakthrough bleeding (and horrible dreams) when I took utrogestan orally. Other people love the sleep it brings, but not me!

The vaginal dose is said to be more consistent so there is more chance of it settling down. It would be really useful if GPs said WHY they suggesting something so we know it is based on some kind of solid evidence or long experience! Otherwise we just have to spend time working it out ourselves and reading up on the research available to the public.
