Utrogestan making me tired

Hi All

I've been on Oestrogel x 2 pumps per day and one Utrogestan for just under 2 weeks.

I think the Utrogestan is making me very tired - and I seem to wake up in the night - not wide awake, but it's not my usual pattern. I'm awake for 30-60 minutes and then I drift off back to sleep. I'm in bed for 9 hours per night in total. It's eating into my day somewhat.

I also just feel a bit crap on it.

Last night I forgot to take it and feel much more refreshed this morning.

I'm wondering if anyone has tried using Utrogestan vaginally and if there is any improvement that way?

Also just wondering how long it has taken for people to feel the benefits of the Oestrogel/Utrogestan treatment.

I'm 51 and nearly 3 years without a period. I started HRT because of joint pain and a baseline feeling of not giving a shit about anything that was not very productive for me.

Hi.
Lots of folks on hete struggle with the utrogestan. Is your dose 100mg every night?

Some women fare better taking it vaginally, in which case they are instructed to use it every other night as more of the progesterone reaches the uterus.

How long have you been on the regimen? Docs generally suggest giving it a good 3 months as our bodies adjust

I had been taking this for almost a year and the last few months I started feeling really tired and my arms and leash felt really heavy when I was in the utrogestran cycle. I came off hrt at Christmas and January as had covid and didn’t even think about hrt. Anyways my anxiety has been through the roof and my doctor suggested I go back on hrt and see how I feel. She also suggested I take 100mg utrogestan everyday for 24 days rather than the big build up over 2 weeks.  I have been doing that for 10 days now and the heaviness is back in my arms and legs. I’m not sure what I should do about it.
Are you taking your utrogestan at night?
I like that it makes me sleepy at night but I really don’t like this heaviness.

Tbh I get the impression that all forms of progesterone are quite likely to make you feel tired, and yet interfere with your sleep. IT just seems to be a feature of the hormone. I've tried a few - couldn't tolerate Utrogestan either orally or vaginally, have ended up on Provera as the least of a bad lot, but I certainly feel much more tired and often nap during the day when I'm on it - I think it may be the price we have to pay for the positive effects of estrogen.

Hiya lindeywalsh - thankyou for getting back to me. yes the dose is 100mg nightly continuously. I've only been on it 2 weeks, but it feels yucky - unless I just need testosterone to counterbalance the Oestrogel & Utrogestan combination, but I won't be getting that for some time (I have an appointment in a couple of months with a gynecologist).
I haven't tried it vaginally but probably will give it a go tonight. The Progesterone is the only part of this regime that I feel is unpleasant. I remember years ago not faring well on the Pill. This is reminding me a bit of that.

Oh dear - that's a bit depressing but good to have a realistic perspective. I think I need testosterone but pretty sure I won't get that from my gp.
At the moment I think the fallout from the Utrogestan is just too much. It's not worth it.

Hiya - yes I take it at night. Then I wake up in the night but in a haze, then fall back asleep and wake up feeling like I've taken a sleeping pill - i.e. in a fog. I really dislike it. I don't have the mental clarity that LIz Earle and Louise Newson talk about (maybe that's becuase they take Testosterone). I take it nightly continuously because I don't have periods any more. It sounds like you were on a different regime initially with the 2 weeks build up.
Yes i experience it as heaviness too.

In this case, every woman really is different, and some fare better on continuous, some on cyclical, some on higher dose long cycle etc. It can really be trial and error. Many clinics say you really need to persevere for around 8 weeks if you can.
I am very progesterone intolerant, so am exploring other options, but many many women do fine a happy medium.

Hiya lindseywalsh - thankyou very much - I've read quite a bit on here and I can see it's a bit of a rollercoaster.
Took it last night and feel like I've been smacked round the head. Plus I was awake from 4.30 and dozed back to sleep unitl 7 whereas I usually get up at 6 so this regime is putting me in bed for an extra hour per day - which I dislike because I'm usually quite busy and productive in the morning. Anyways - enough of my moaning. I will persevere and it's good to know that I should keep with it for 8 weeks.
By the way dp you happen to know if taking it vaginally can reduce the tiredness? I know Louise Newson recommends it every other night vaginally @ 100mg so I thought I might try it.

I would say it is definitely worth a try. At Newson their view is that much more (approx double) the progesterone reaches the uterus when taken vaginally, hence only needed every other night. Also the progesterone bypasses the liver, so that’s a plus.

I have read on here that one lady stuck with the programme for 7 months and eventually the side effects reduced to a much more comfortable level.

Remind me - are you perimenopause or post menopause? If the latter, your own hormones are still fluctuating, so a bit trickier. Ie you may be supplementing with progesterone at a phase when you own progesterone is having a bit of a surge.

The general rule of thumb is 10-12 days of 100mg vaginal utrogestan as a minimum to protect endometrium lining. Some women have therefore spaced out their progesterone intake to every 3rd day over the month, which seems to work well.

One other option used by Newson is to use cyclogest. This is a fertility progesterone, but essentially same as utrogestan. It comes in a 200mg waxy less airy. It can be cut in half to equate to the 100mg utrogestan, and has suited some women who don’t do well with the utrogestan. Some women have cut in 1/4 and therefore taken 50mg each time.

For many women, the dose of progesterone available is just too strong for their bodies, hence the trial and error of different regimen to introduce enough progesterone to protect endometrium.

I am still using trial and error ..... looking at trialling tThe cyclogest regimen next, and also investigating HRT compounded progesterone, which is available in much smaller doses, to see if I can figure out a level which works for me.

Hiya again Lindsey - I'm post meno - coming up to 3 years no period. Do you mean that my hormones will be fluctuating - I thought that was when you're peri?
Yeah I saw Louise Newson talking about the Cyclogest - I'm happy to use the Utro and to try vaginally every other night.
It's not horrendous, not yet, but it's also not great - I can feel that something unfamiliar and foreign is in my system and if I'm on HRT to counteract the fatigue, then it seems a bit silly to be tired from the HRT - No such thing as a free lunch in my case ;) Damn these hormones.

I'd be interested to know how you get on too with your experimentation too.
