Swapping Utrogeston for Mirena Coil
« on: January 09, 2019, 08:50:41 AM »

I am on 2 pumps of Estrogel a day and take Utrogeston for 12 days a month which is meant to be days 15 - 26. The thing is my periods are anything from 22-26 days long so taking the progesterone for those days is out of sync. I'm not sure if there is anything I can do about this. I've been on this regime for 4 months. For the first 3 months I took the Utrogeston for just 9 days and then my Doc upped it at my request as I believe this is the correct dose. I am having a really bad month 4 though. I am 1 or 2 days before my period I believe due to my symptoms and I have been taking the Utrogeston for 9 days now and I just feel so low and anxious and I cannot sleep. I read on here that people are progesterone intolerant and my GP did mention this about myself but I don't know what this means and if it refers to how I am feeling.

My GP has mentioned swapping me to a Mirena coil and seems to think that this will improve my menopausal journey a great deal as it will reduce my periods (which sometimes are so heavy and long I cannot leave the house).

I am interested to know how other people have got on with swapping to the Mirena and if it has alleviated any of the nasty symptoms they were having, or made it worse!

I couldn't tolerate utrogestan, the sleep  disturbance was awful and I switched to the Mirena, I haven't looked back!

I'm really gutted that my body is not reacting well to the Utrogeston.

Tinkerbell, has it relieved any PMS symptoms or just made it all more tolerable in general?

Hello AngsanaBlossom,

Maybe it's just your own hormones playing up. Oestrogel plus Utrogestan doesn't seem to be enough to override perimenoupause hormonal ups and downs. I also had an awful time in peri and my docs suggested the coil but I just couldn't face it. I stopped HRT and waited for the menopause. Things have calmed down dramatically regarding anxiety, joint aches, sleep and mood swings. Still having hot flushes but they are nothing compared to the other symptoms.

If you really need HRT, maybe give more time to this regimen to settle or endure the coil.

Hi AngsanaBlossom

You may have forgotten (common in menopause!) but you asked the same question a couple of months ago so here is your thread where you got some answers:
https://www.menopausematters.co.uk/forum/index.php/topic,41945.msg660601.html#msg660601

What were your periods doing before you started HRT ie were they regular or had your cycle length started changing? It sounds like they had become shorter - is this what you mean by the 22-26 days - or is this after the HRT? You may not yet be properly in the peri-menopausal transition if this is the case (regular periods with shortened cycle) - if you are still ovulating regularly. Hormones begin to go awry during this stage but it can be difficult to treat. In younger women some gynaes recommend the contraceptive pill to regulate the cycle and prevent the hormonal surges, but they do contain progestogens of course - how old are you?

The Mirena sounds like a good option - less progestogen is absorbed systemically than from a contraceptive pill conrtaining the same type so you may well tolerate this. I didn't start HRT until I was late peri-menopause which was ideal for me as it worked like a charm and my own hormones were not so active and heavy bleeds had stopped.

Hope this helps.

I'm about to do the same because I bleed almost all the time on continuous Utrogestan and I have a skin irritation that is much worse since being on the Utrogestan plus loads of fluid retention.  It's good to see your comment.  It gives me hope.

I couldn't tolerate utrogestan - anxiety, bad dreams, heartburn. I tried mirena but unfortunately I spotted every day for 6 months so I had it removed. On the upside, I had no other side effects so it may be worth you trying. I'm now on provera for 12 days and it suits me better. I do sometimes have the same issue as you that my cycle is short so I end up starting P again before the end of the bleed so I'm considering trying continuous P soon.
