So if I'm having a bleed now I would use the Estragel first for 14 days then the Utrogestan? Do I wait for my bleed to stop? Are you using Estragel too? xx JG

I take 200 mg utrogeston vaginally last 12  days of a cycle ,I have the pessary soft capsules perscribed off licence by private consultant,but the 100 mg caps can be used vaginally as well. Not sure if 100 mg for 12 days will be enough to shed the lining properly or not though,hopefully Hurdity will be along to guide you.

Yes you use the estrogen for first 14 days ,this causes the lining to form,then use the estrogen and utrogeston for 12 days,then you should have a bleed.

It's tricky counting days when you are swapping essentially from a conti regime to a cycle. Ideally if you count your bleed as Day 1 then Days 1-14 you take oestrogen only as others have said, but if you are having a bleed at the "wrong" time and you are trying to re-set them then you'll need to change when Day 1 is if you see what I mean? What day are you on now and are you taking Oestrogen only? Yes just as you said in your post - ie the first 14 days but day 1 must be the first day of bleed otherwise you will still be out of sync ( but you could well be anyway, as you are in peri!).

Re the preparations - the discontinued 200 mg Utrogestan capsules and also the 100 mg ones are licensed for oral use but which many of use use vaginally - same product. The 200 mg Utrogestan capsules for fertility (vaginal use) are still being produced but they are exactly the same as the old 200 mg HRT Utrogestan - so I can't understand why the others were discontinued since it's only the box and the leaflet that is different!!

As has been said the licensed dose for HRT is 200 mg for 12 days per cycle but off license some gynaes will permit variation from this and especially if used vaginally. I use what I calculate to be half the amount vaginally that I would need orally for the amount of oestrogen I am having. This would need supervision though to ensure no endometrium build-up

My bleed came 5 days ago but was a breakthrough bleed anyway after not having a proper bleed after my last Utrogestan break.I used only Estragel last night so guess that could be my day 1?
I would have to see GP re the 2oomg Utrogestan as have 100mg at the mo.

I use 100mg Utrogestan for 12 days of the month and use Estrogel one and a bit pumps every single day. That brings on a nice bleed and keeps the lining thin.

this is my first cycle on estrogel/utrogestan. I used estrogel (1 pump) and utrogestan 100 vaginally for 12 days. No bleeding after 2 days, just achy and have a slight temperature. Is it normal not to bleed on the first month during this protocol?

Hi foreign body - that is quite a low dose of oestrogen so depends where you were in menopause before you started the HRT. If your periods had become very infrequent and your cycle only sporadic, then your own oestrogen dose is probably very low, so with a low dose of gel you may well not get enouh womb lining building up during that time. For the 12 days you are taking the Utro also there is no build-up either. Sometimes the bleed takes longer than two days to arrive for some women, and sometimes it may vary in amount if your own cycle kicks in from time to time. Nothing to worry about! It shouldn't give you a temperature though? Perhaps you have a bit of a bug?

thanks Hurdity! According to the blood tests/scan and my ob/gyn i am already menopausal. I haven't had a period for over 8 months.
One estrogel pump a day is enough to keep all the symptoms under control, that's why i didn't  increase the dosage. I feel great on estrogen but awful on progesterone. I am going to try estrogel only for a couple of months and then take progesterone again and see if i get a bleed. In September i will go for a lining scan.

Hurdity - if you read this firstly sorry if you answered a question I posed few weeks ago and I didn't answer, work has been manic and when I quickly looked through I lost the link!   As you know I have chopped and changed over last few years of utro use, just wanted to update that as I didn't have the bleed last 2 cycles on the 100mg utro using it vaginally for the (I think it was 12
Days) I am now taking the plunge and have started using the utro at 200mg vaginally for 12 days, I am going to use this starting from the 1st of the month,    As you know (I think) the utro gave me extreme tiredness which I couldn't cope with work wise before, that why I was changing around regimes, anyway I started utro 3 nights ago and so far ok, so 9 more days to go! I will see if I get a bleed and keep you posted!

I'm on 1mg estrogen and use 200 mg utrogeston vaginally for last 12 nights if this helps you. It Did make me a bit tired and I didn't feel great on it but as I do the long cycle on it I guess I will have to grin and bear it as all other progesterone are intolerable.

just an update. 4 days after stopping the progesterone i started bleeding, which means the protocol is working :-)

I'm sure it will come if it needs to! For some women the bleed takes longer to arrive. Do not fret  :)
