Sandrena & Utrogestan

Hello
im hoping someone on here can help me.
Ive only just started HRT and im on Sandrena sachets and Utrogestan tablets.
My GP has advised me that when I start the tablets on day 15 to take at the same time as I use my gel and 2 hours after food and before bedtime. however I don't feel as this is going to work with my timings of things. We only eat at 19:30 most nights and soon after eating and tidying up we generally go to bed. Is there a reason why you take the tablets at nighttime?
Also , what do you do when on holiday ?

Hi there

I am on 1x 100mg Utrogestan and 2x 1mg Sandrena daily, no break

I use Sandrena on a morning and Utrogestan on a night at least 2 hours after eating food.  Reason they advise for taking Utrogestan on a night time is that it can make you sleepy, even more so if you take it with food - you can get used to it to be fair - although we are all different and can react as a result - hope this helps 👍

Taking Utrogestan with food increases the absorption.

I've never heard of transdermal or any other oestr needing to be taken at same time as utrogestan - that said I've never done any gel let alone sandrena.  Plenty of people here put gel on morning and evening though, or change their patches every 3.5 days as I used to do, so it doesn't sound right - unless GP meant easier to remember!

Utrogestan can make you drowsy, so night-time definitely.  It is best taken 2+ hours after food so on an empty stomach - that's to do with absorption.  I think I read somewhere that weirdly food actually increases absorption of utrogestan (which is the opposite of what you'd expect) - and when you're starting out you especially don't want to be absorbing excessive (or taking it in morning!).

That said, the leading meno consultant I'm very lucky now to see said when I asked him - for me it's challenging for some of same and some different reasons - said ideally 2 hours (which is in itself obviously an approx - many medics suggest a bit over 2 hours for empty stomach, and it varies from person to person) but he wasn't going to lose sleep it if was a bit less, but just not to take it on a full stomach (or too close even before to food).  So fwiw - and I'm not a doctor - I'd suggest definitely at night, as long after food as you can (just before lights out!) and don't stress particularly on holiday if sometimes that's a bit under 2 hours.

Good advice.  Many medications are dependant on whether to eat/not with food.  It enables the drug to do what it's designed to do.

If you take it vaginally it doesn't matter when you've eaten. For most people it helps with sleep but it disrupts mine so I take it around 11am. Best to try it and see, if it doesn't make you drowsy you have more options for timing.

Not here to argue but fwiw my consultant said def night-time and ideally 2 hours both apply whether oral or vaginal. Systemic absorption is less on vaginal but not zero - and highly individual

Did he explain how the contents of your stomach were going to affect absorption from your vagina? If you use vaginal utro for IVF the advice is only to take it before bed because for many women it had a sedative effect. There is no restriction concerning food. If you're one of the women for whom it disrupts sleep taking it before bed isn't great timing, it's better lf it's worn off a bit.

Not here to argue or hijack a thread. I’m not here for dogmatism or anecdote - I’ve been very lucky after shockingly bad ‘advice’ that made me very ill to see an actual leading expert and I’m here to share what I’ve learned.

I didn’t cross-examine him; I’ve other priorities when seeing a consultant who’s had over 100000 academic citations, world leader in HRT, and has already undone some of the damage done me by private GP ‘specialists’ who definitely know more than most GPs and some general gynaes (not a high bar lol) but are better at PR (and charging consultant prices) than they are at understanding actual medicine.

Absorption is highly individual- as anyone reading Crispy’s thread has recently been reminded by Angela.
