I have an appt next week with a trainee GP I've never met before.
I want to increase my estrogen and want to be prepared for potential battle.

The reason i want to increase estrogen is to control mood swings and headaches, but I have had raised blood pressure - (130s and 140s)

Background: I'm 53, so tail end of peri. I have had Mirena since Feb 2025 and Evorel 50 (which I've taken as part of sequi regime for 7 years). Headaches have significantly increased, but everything else has been better. Mood swings still occasionally.

Any pointers for me such as research or pitfalls would be much appreciated. As well as any similar experiences.

Tail end of peri ?  Peri are the years well B4 the final period = menopause ; when we being 2 notice various odd symptoms, i.e. periods wax and wane; headaches may become worse as can anxiety and depression.  We are advised that we may be into menopause when we have been period free for 12-14 months, however Mother Nature can throw in another bleed when we have to begin counting. Again  . 

If using HRT we cannot know where we might be in the journey towards menopause.

Were your headaches cyclical during your menstruating years.

I was still having irregular periods until I had mirena, but since then I've only had spotting. But I still get some cyclical mood swings.

I had cyclical hormonal headaches previously, which have sig increased since having mirena.

I am anticipating medic saying I cant increase evorel and want to be prepared with rebuttal.

I believe that there is a new drug to help with menopausal migraines, it might be worth while doing a search?

You're well below the nhs limit of 100 so you should be OK. Just stress the meno symptoms you have that aren't or are no longer controlled.  They should know your own production is decreasing.

I have tried to find the bit about 100mg Estrogen in the NICE guidelines, but can't find it.

Can anyone guide me to link about dosage levels, so I can go armed to my appt?

Fusseh, a 50mcg Evorel patch is a very low dose and unlikely to be much help.  I had a Mirena coil and a 50mcg patch and it was completely useless because it often didn't stick properly and I was absorbing a pitiful amount of oestrogen.

I would ask for a higher dose (100mcg if you can) or switch to Oestrogel which I found much stronger and more effective.  Although in theory the Mirena coil provides localised progesterone, enough of it gets into your overall system to cause side effects like headaches and you need a much higher dose of oestrogen to counter that.
