No bleed on sequential hrt?
« on: November 01, 2025, 10:48:11 PM »

Hi

I've now done just over 2 months of sequential hrt, two pumps of oestrogel daily and 1 x 200mg progesterone days 14-28. I'm also on the progesterone only pill. I was only have extremely irregular bleeds prior to starting (basically a couple of bouts of very light bleeding for a few days at a time over the course of maybe a year) before starting hrt. So far since starting hrt I've not had any bleeds at all. Has anyone got experience of this?

Why would you take something to stop you bleeding (the POP) and another thing to make you bleed (sequential progesterone)?

That's Alice in wonderland medicine!

You would be better either using a double dose of the POP (assuming you are on desogestrel) ie 150mcg daily as your endometrial protection (off licence but supported by the BMS based on available evidence) and ditching the micronised progesterone, or continuing single dose POP with 100mg progesterone daily.

Either way, that much progestogen is a high price to pay for one pump of oestrogel.

If you don't need birth control or bleed control, you could just use the micronised progesterone.

Thanks, I was thinking of asking my GP about switching to continuous progesterone so I think I'll go back to her and ask for 100mg daily (my prescription is for 200mg capsules so I can't split the dose myself).

I'm still trying to get my head around all of this!
