Too much or not enough?

Morning guys, I started Zoely a few months ago and it's saved me from crippling PMDD. But I've had lots of facial tingling. It feels like ants are crawling beneath my skin. Is this a sign of not enough of one hormone or too much of another?
Also, although I take the packs back-to-back, I still feel a bit crampy/emotional at times and as though my cycle is still running along in the background. The PMDD is MUCH better, but I would've thought Zoely would have shut my cycle down altogether?
Finally, does anyone know what the HRT equivalent is to taking Zoely (i.e. one pump/two pumps etc.)?
As an aside, I have the breast cancer risk of it all at the back of my mind. I know the risk is small and we need to weigh up quality of life etc, but still...

I take Zoely too, and I would be very surprised if your ovaries were not completely suppressed if like me you take the active pills only, because of the highly targeted and relatively kind progestin NOMAC.

An ultrasound scan can confirm this.

Ovarian suppression does not necessarily completely rule out brain symptoms, and if you are under the care of or have access to a specialist, some would sanction a small additional dose of estrogen say from a patch or gel if you feel the estrogen dose is insufficient - it is relatively low at 1.5mg however my plasma levels have always been adequate (around 400pmol/L) on it although I do have high ovarian reserve and lots of remaining dormant follicles which still trickle out estradiol even whilst suppressed, so this is something I plan to monitor as I get into my late 40's and 50's.

There is no licenced HRT product that comes anywhere close to Zoely as these are not designed to shut down an endogenous cycle - if you have PMDD it would simply come back without a suppressive progestin unless you actually are postmenopausal.

Additionally micronised progesterone is the current go to for endometrial protection with menopause hormone therapy, and this, particularly if given cyclically, is in the words of the late professor Studd and with which I agree, a pharmacological model of PMDD.

Do what I would do is look into supplemental estrogen under the care of a specialist, and also think about your androgens - Zoely is so good that because your ovaries are in hibernation, you will have very little testosterone.

I take DHEA for this although you may be able to find and afford someone who would be willing to prescribe actual testosterone - I believe Louise Newson' would consider this in someone with a medical indication for being on the COCP.

Out of interest, have you ever experienced the tingling I describe? I think it must be hormonal.

Can anyone advise?

I have personally never experienced this but it is called formication and is a recognised symptom of peri/menopause and frequently responsive to estrogen treatment.

If the "formication" affects your face only, it sounds a lot like the sensation I get during a flare up of rosacea.

Thanks. It mainly affects my face, but I get tingling in my fingers sometimes too.

Just wondered if maybe the oestrogen in Zoely is a little low for me.

It's all a bit of a minefield.

Hi Gillian x I also have PMDD 😫

I don’t have tingling but I do have facial pain (nose, palate, teeth) which was diagnosed as sinusitis and which I had FESS (sinus) surgery for.

The surgery doesn’t work and it seems the pain is part of hormonal migraines…the pain can feel tingling/stingy at times though x

Sorry to hear, Sue. That sounds horrible. Hormones have a lot to answer for.

The pill is definitely helping the PMDD, so that's something. Might just need to put up with the tingling as a trade-off.

They really do, Gillian!

Yeah, I’ve had a nightmare! All caused by mirena and Evorel 100 to treat PMDD!

I’m on Prostap now, with addback oestrogel and utrogestan.

I’m about a week after the second injection and the fatigue is still grinding.

I’m wondering if it’s the utrogestan now as I seem to perk up late at night when it’s due 🙄

Goodness only knows what the answer is!

If I’m as bad with next jab (48 hours in bed and a week of debilitating fatigue and myalgia), I think I’ll have to find another solution…I can’t keep doing it to myself and need to be well enough to work, which I haven’t for three months 😩
