Tranexamic acid - yes or no?!

Hi everyone, I'd be grateful for any advice. I'm on day 41 of my latest period. Two months prior to this one I had one that lasted 20 days. The doctor prescribed tranexamic acid and it stopped after 3 days. I was referred for ultrasound which found no fibroids etc, just small endometrial thickening - 6cm. I'm currently waiting for my first gynae app in two weeks time.
I am just very nervous about taking the acid again - just don't like the idea of it clotting my blood throughout my body! But I'm exhausted, have a major work event coming up and would love the bleeding to stop. Help! Also would love to hear about similar situations. I'm 55 and periods only became irregular at the beginning of this year.

Hi

I'm now post menopause, but I took tranexamic acid for years for heavy periods.

It was most effective if taken with ibuprofen, and just before a period was due (to get into your system and start working). However, there is a maximum number of days you can take it for at any given time (not sure why), but can't remember how many days exactly.

Personally I am not a fan of tranexamic acid - I don't see the point of telling women they can't have the birth control pill to get shut of their periods because of a minuscule risk of blood clots, then prescribing something whose primary mechanism of action is to induce indiscriminate coagulation.

However it does have a role in some situations and can be highly effective as a short term treatment.

In your situation I wouldn't be afraid to take it short term if it has worked before and you have an event coming up, however I would also be looking to get on a progestin for longer term control.

Oral progestin treatment from your GP (such as provera, norethisterone or a double dose of the desogestrel mini pill taken daily however they may not favour the last one unless particularly experienced) would be best in the meantime pending gynae review - you could also consider an IUS, this is likely to be discussed at your appointment.

Unfortunately I'm progesterone intolerant so that's why I'm not on hrt. So yes TA may be my only option  🫤

I'm just hoping to hear from someone who may have had extended bleeding like this, without something more sinister than hormone imbalance being the cause. 🙏

Hi.  I thought that this was used for heavy, prolonged bleeds ............  :-\. If I had an event in the offing I would take it.  My periods were heavy, excruciatly painful and would last 10 days: then a break of a few B4 another 10 days.  For months. No-one suggested that I could take something to ease symptoms  :'(  :-\

When you say progesterone intolerant, what have you tried?

It is very rare that someone cannot tolerate any progestogen.

In the vast majority of cases problem bleeding is non-malignant.

There are other options for you after you see gynae such as endometrial ablation.
