Oral combined hrt and combined patches?
« on: August 18, 2025, 09:52:46 AM »

I am 56 and deemed to be post menopausal.  I was recently changed from Femoston 2/10 to evorel conti 75. I definitely feel like this needs to be increased at my 1st review. I was very happy on the Femoston, but in the last few months started to feel like I needed something stronger. I am progesterone intolerant so have less options if I need increased estrogen and was wondering if it was possible to have continuous oral hrt as well as continuous hrt patches

Hi Paula

The great news is that the progestogen in Femoston - dydrogesterone - is now available on its own as "Nalvee" so you could take the oestrogen and progestogen separately. The oestrogen could still be delivered via patches. There is no need to take continuous combined HRT just because you are post-menopausal, if you were happy using cyclical HRT. You can remain on cyclical HRT due to progesterone intolerance. You could have an Evorel patch 75 mcg along with the 10 mg Dydrogesterone for 12-14 days per month - and duration could be increased if not effective in protecting your endometrium.

The GP probably won't have heard of it yet (after all it's only just been launched) but if you look at my recent post titled "Nalvee" - you will see it is listed on the Medicines website....

Hope this helps

Norethisterone (same as in the evorel patches) is available as an oral tablet too. The drawback is that it's only available in 5mg when you only need 1mg. I cut them in half when I used it but it's still more than you need.
