Feeling fed up and depressed!

Hi everyone.  Those of you who have read my posts before will probably think I’m being very impatient, and I know it’s not my strong point having ADD & low level OCD but every morning I wake up and it’s the same thing - for a glorious few moments there is no pain and I feel normal, but then it starts - before I’ve even moved! How does it know I’m awake?? The pain, burning and stinging - regardless of what I put on it, it’s set in for the day .. it’s just so painful and depressing .. a few weeks ago I had days where it would ease up for a couple of days but not anymore - well not since I started to do more during the day anyway.  I haven’t been able to work since this started on 12 April.  I’ve been on estrogen cream since 20 May and my patches which were 25, then 50 have now just been increased to 75mg and I use vagifem every day.  Yes VM & sometimes Vaseline.  Those of you who have found some relief from GSM - was your recovery a slow but consistent one? or was it a case of some good and some bad days?

I’m back to see the gynaecologist in a couple of weeks’ time but I don’t feel much better than I did when I went in May… she said I was ‘complicated’ as I also have lichen sclerosus but I’ve had that for years and it’s never bothered me.  She said the pain might be more neuropathic as when skin is damaged the nerve endings are exposed hence me being on gabapentin & amitriptyline but sometimes I need oramorph when it gets really bad, like after driving (even with a donut cushion) I’m sorry I know I sound I’m moaning and I know there are women worse off, but sometimes I just don’t don’t want to wake up in the morning …

With this condition it's all about patience when first starting with the treatment. It took me four months to start to feel relief with daily Vagifem and Estriol and a year before it was under control. I have straight forward atrophy but when its complicated its going to be a rocky road of trial and error with treatments, thankfully you have a gynaecologist looking after you and hopefully you will both find a solution before too long.

Thanks Ayesha, for your words of reason and for all your help so far.  I did try a little of the YESVM with the estriol cream and it was less stingy. 

I think one of my ‘problems’ is that having been wrongly diagnosed and mis-treated three times I keep thinking have they got it right now? Is the estriol the right thing to use? I know it must be as the atrophy wasn’t a complete surprise as I’d been off systemic HRT for 2 years, using vagifem only since then.  When I first went on HRT I was given ovestin but it didn’t work that well, so I went on to systemic HRT which did the job perfectly, so I’m hoping that I’m now on a reasonable dose patch it’ll kick in soon.  I’m not near 4 months yet so I’ll try to be more patient.  If I fully trusted the gynaecologist I’d be happier - though I’ve no reason not to really - she seemed pretty certain of the diagnosis,

With any health condition I think it's normal to be impatient for treatment to work. I'm not sure it's being impatient rather than that  after so much pain for so long we just yern for it to go & quickly.
I had a gynae about 6 years in after repeated GPs said they had no idea what was causing the pain. I'd actually given up going to them,  can you believe, because id be sent away with dermovate which just made my pain worse. It was only because my husband phoned the GP & had a furious conversation with them about them doing nothing that I was referred to this gynae. She diagnosed me with a few different things I didn't have & refused to consider I could have symptoms caused by menopause. She got angry with me because every time her treatment didn't work, I asked again, could this be menopausal.
So it's hard to trust the professionals. I've been told by my GP I have lichen too. Given dermovate which will just damage my thin sore vulva & not had any more than a 'sight' diagnosis.
Like you I'm not confident that I have a complete diagnosis or correct treatment. It took 6 months of daily pessaries & cream for me to not notice any pain & occasionally have sex. But it didn't last more than a few months. I do know a gynae often doesn't know a thing about how menopause affects that area. I can't remember from your other posts, have you seen a menopause specialist? Also did I add on one of your posts the suggestion of lidocaine 4%? (My memory is bad!) because it is a massive help.
I think you are early days but both daily cream & pessaries for months was the only help for me. So surely you need that too?
It does seem lots of us are told we have multiple conditions in my case, I'm not convinced. It is waring having no trust in who we see, either. It's concerning you aren't able to work, either, and you say things are worse because you've been doing more. That's just not right. You shouldn't have to spend your life doing nothing because of the pain.
Are you able to have a private consultation? I'm at the stage where I'm considering it. Or do you have someone to advocate for you with your GP or gynae so they can be firm about how your life is affected? I've found that I'm so worn out with all this I sometimes just accept what I'm told.
I do feel for you, literally too! We should be able to get what we need but often don't. But persevere, if you get enough dosage, it should work or at least if not then perhaps it would indicate other types of GSM meds should be tried. I think you've seen the link to the different options on her.

Hi.  I’ve also sent you a PM before I saw your reply on here.  I’m sorry you’ve had such a battle to be ‘heard’ by the medical profession.  Low estrogen due to menopause isn’t a rare condition that most medics can’t treat - it’s extremely common and probably affects most women at some point so why all the misdiagnosis and lack of awareness and compassion? It’s just awful!

Unfortunately, as I live on an island, there is no private gynaecologist nearby.  I can’t face the travel to find one on the mainland. 

I have a friend who’s been so helpful phoning people on my behalf (she used to work in welfare so she knows who to speak to and what to say) she got me a gynae appointment in 4 days after my GP told me it would be a year’s wait! (and that after I’d been seen in A&E due to the pain!)

It’s awful that they gave you dermovate too - yes, it’s the right treatment for lichen but not if you have thin skin - that needs to be addressed before they start to work on the lichen.  I just get cross at the way they give you the wrong treatment and then imply it’s your fault when it goes wrong! She said to me ‘oh - you shouldn’t have been using dermovate! You’ve made it so much worse!’ So I said ‘well it was your department that gave it to me!’

I’m glad the cream and pessaries eventually gave you relief - I’m encouraged that I might get that relief too albeit in a few months time, can I ask what it felt like for you? Mine is mainly one sided, and it feels like a burning,  stinging, soreness that is made worse by sitting (I can hardly sit at all - maybe just on the side of the bed when I get up slowly as I gave vertigo) - no throbbing or shooting pains, just the soreness.  Estrogen cream stings but I can cope with that as I know it’s the only thing that will heal the skin.  It’s uncomfortable after I’ve been for a pee - due to wiping, no matter how gentle I am - although this has improved slightly. I’m fine overnight and if I wake up for the loo it’s fine - I feel quite normal.  It’s just the daytime that’s such a challenge!

I’m sorry you’ve had all this too! Who knew when we were young girls that all this was to come?

Ooh that sounds awful.
I never heard of lichen sclerosus until earlier today but I found a link with reliable info https://www.drlouisenewson.co.uk/knowledge/vulval-lichen-sclerosus-and-menopause
I'm also experimenting with vaginal hormones, vagifem and estriol, I am considering using the DHEA cream as it has both hormones we need down there.
Have you had your oestrogen blood levels checked?
I ask because I never, for years, and it was only in March this year I found they were 💩 at only 112pmol and I was at 75mcg then. I have had my dose increased since then.
I only found out I was a poor absorber of transdermal oestrogen, in June after another low blood test 289pmol and three increases to 150mcg.
I am still trying to get to that target range of 400, currently on 225mcg.
My GP and specialist think this is an insanely high dose, however if not much is getting through to the skin, then it isn't getting into the bloodstream.

Goodness! No, I’ve never had my levels checked - I didn’t know about that! I have blood tests all the time for other things but apart from a FSH test to confirm I was in menopause I’ve had nothing else.  I’ll ask the gynae people when I have my next appointment.  I know that systemic kliofem worked for me 2 years ago but now I can only have the patches - I think, if I’m correct, that the Estraderm 75 is a similar level of estrogen to the kliofem …

It’s all rather complicated isn’t it? I’d like to know how well I’m absorbing the estrogen from the patch though … sometimes they go a bit crinkly (like me I guess!) on my bottom.  Got nowhere else to put it though as I have daily injections in my stomach.  Thanks for the info - that’s very interesting!

Snowcat, mine is the whole vulva & particularly the entrance to my vagina. It feels like I've pressed a hot iron on it to actually cause a burn. It's also internal but I'm not aware of that unless my husband & I attempt sex.
I can get stabbing pains but it's more the constant burn.
Because I do a manual job I have to wear sensible shorts or trousers & have to be very careful about the stitching on the crutch area of them because too much pressure, particularly in the hot weather, can cause either a tear or a swollen lump to come up around the vaginal entrance (sure there's a name for that bit!). So I can look a bit misformed at times in that area. Ice packs are often the only relief but obviously only once I get home.
I genuinely don't know exactly how many conditions I have or whether a lot of it has been caused by the dermovate. Not fun, eh ladies?

Aw, that’s awful - you poor thing! Makes my symptoms seem mild! I can’t bear the thought of having to wear shorts or trousers! I’ve put mine away and bought a few dresses (I hadn’t worn a dress since my wedding day!) that must be so uncomfortable for you!

I can’t believe the struggle you’ve had to get help when you’re clearly in pain! I do hope things improve for you soon.  x

Snowcat, no practical help here from me, just a hand-hold and sympathy. Sorry you are having such a tough time. I hope your next visit to the gynaecologist helps and, if you feel comfortable with the idea, I think MrsMitch's idea of having someone trusted with you for support is a good idea. I realise that may not be possible.

Thank you Sunnyangel, that’s so kind.  My husband will come with me although how much help he’ll be I’m not sure 😂 he’s not great in medical surroundings! My friend might come but it depends how she is as she’s very poorly.  I will write down everything I want to say - I know this alarms most doctors but I don’t want to forget anything as gynae appointments are as rare as hen’s teeth!
