Hi, I’m switching from Evorel Sequi to Tibolone, and my consultant was very vague about how best to do it! Should I complete the current cycle of patches, or just switch straight away? Or not sure if I should have a break of a few days inbetween. I’ve had a google but not much came up. Any thoughts or experiences would be appreciated. Thank you!

You're overthinking it.

I'm assuming the switch is due to the sequi patch either not being tolerated or not being effective.

Therefore why would you not just take the first tibolone pill the day you get it and remove the patch.

Thanks bombsh3ll. Yes, I’m not feeling good on evorel sequi. But, when I asked my consultant if I should leave a gap between options she said, “oh, hadn’t thought about that, maybe a few days to let the oestrogens drop”. It was such an afterthought, I goggled to see if there was a general consensus, and saw on here that those who went straight on sometimes seemed to have more side effects. Another website suggested finishing a whole cycle of the patches before changing.

Very difficult to know what to do when no one agrees. Or, as you say, I’m overthinking and it doesn't really matter.

It doesn't matter a jot in the grand scheme of things but short term will be more comfortable for you if you don't leave a gap, otherwise you may suffer abrupt estrogen withdrawal symptoms.

Your consultant doesn't fill me with confidence either thinking it's a good idea to "let estrogens drop"!

Regarding the suggestion to finish a whole cycle - there's no physiological basis for this. What does anyone think might happen if you just start it now?!

I think this type of idea comes from old fashioned advice about when to start the birth control pill to ensure the user isn't pregnant 😉

Worst case you might get a bit of bleeding, but this can happen with any switch.

Unless you're early on in the evorel cycle personally I'd wait for the bleed so you start off with a thin lining. I don't see the point in taking a break from oestrogen though, that's better with a consistent dose.

Thanks both, that’s really helpful. I’m mid cycle right now. I’ll do a couple of the patches with progesterone next week, partly as I have a busy work week and don’t want to be dealing with any temporary side effects. Then once I’ve started my bleed I’ll switch straight onto the Tibolone.
