Combined patch plus mirena - thoughts
« on: July 31, 2025, 09:13:07 PM »

Hi all, I started on HRT about a month and I half ago so I know it's a bit early to be sure about the specific regimen. I am just wondering if the one I'm currently on makes sense. I'm 41 and the GP felt I was symptomatic for perimenopause and prescribed hormone replacement (she's very sympathetic, has advanced knowledge in menopause). My main symptom was irritation/rage and she said she prefers a continuous dose where everything can be a bit more evened out. Which makes sense to me. I suppose I'm just wondering do I really need progesterone in a patch if it's also in the mirena? When she first prescribed, obviously I needed combined. As she was recommending the mirena but she really didn't know when I'd schedule it etc. But now that I've got it, I wonder might it make sense to lose the extra progesterone? I had felt an immediate improvement in another symptom (low libido) on starting the patch. Upon having the mirena fitted that has dropped even further through the floor. Will be back to doctor in a couple of weeks but I'm just interested in anyone's thoughts or experiences.

You're right, you have too much progesterone now. You should have oestrogen only patches.

Thanks ladies. I'm hoping when I go back she might change that, rather than waiting to see how things pan out. I just don't think it is going to change as years ago before I had kids, I used progesterone only contraception and that too killed my libido!

It was reasonable to prescribe the combined patch given you didn't know how long you would wait for the IUS.

However now that you have it there's no need to double up on progestins, and you should change to estradiol alone patches.
