Hello, I'm new and have just found this forum.

I'm almost 55 and had Merina coils for past 15 years. I had no bleeding for 9 years since the last coil but blood tests confirmed menopause. 

I had the last coil removed 18 days ago at the same time as my smear. I did not bleed initially (it started the following day) but have been bleeding ever since.

When I started passing 5cm clots, I was prescribed medroxyprogesterone 10mg 3x a day. I'm no longer passing clots but I'm still bleeding non stop day and night. It's very inconvenient and interrupting my day to day life.

I've seen a Dr who does not know why I'm bleeding. My scans 'look ok' (no uterine thickening) and my smear was HPV negative. He tried to do a hysteroscopy but failed so I'm being referred for possible spinal block so they can try again.

48hrs ago I started to get severe gallbladder discomfort (I have gallstones but it's managed by diet with no pain for years). I know this is a complication of the medroxyprogesterone so I have stopped it today and been prescribed Desogestrel 75mg 1 per day. But, I've just read in the leaflet 'do not take if you have gallstones'.

I'm at the end of my tether. Until 18 days ago I had no menopause symptoms and no bleeding. Now my life has been turned upside down just by having the 9 year old Merina coil removed (whose hormone would have finished after that time so should not have affected me).

I'm desperately looking for guidance about what to do next.

Desogestrel is unlikely to cause a problem with gallstones and the treatment dose for bleed control is usually 150mcg not 75mcg daily.

However it would be reasonable to try an empirical course of antibiotics for possible endometritis as well if this hasn't already been done.

I would give medical therapy a longer chance to work before rushing to invasive investigations if you have no risk factors for endometrial hyperplasia and weren't using an expired IUS for endometrial protection alongside estrogen.

With prolonged history of IUS use your risk of endometrial cancer is significantly lower than the average woman as the strong progestin is highly protective.

Trying to understand all your comments as this is a brand new topic for me.

Are you suggesting that the Desogestrel should be fine for me to take? Also that I should ask the Dr for antibiotics too?

And that perhaps I should delay the hysteroscopy?

I'm completely in the dark here but I need this bleeding to stop. I've never bled for 18 days before and I really don't know what to do for the best.

Whilst I can't give you medical advice, if it were me in that situation I would be taking 150mcg desogestrel daily until it stopped and also getting a course of antibiotics to cover possible endometritis from foreign body removal.

Given the normal endometrial lining on the ultrasound scan I personally wouldn't have a hysteroscopy unless either I had risk factors or it had gone on for at least 3 months.
