HRT increase

Hi everyone,

I am currently on Evorel 75 patch and 200mg progesterone pills sequentially. People have probably seen my other posts. I increased to this level in March 2025, and on my 3 month review in June I was still happy with that so my GP has kept the prescription the same. Anyway about 2 weeks ago I started to get random joint pains as I’d had previously at the start of peri, which included stabbing pains in the middle of my back, aching shoulder/neck, sore wrists, night waking and also burning feeling in my cheeks, which I’m attributing to low oestrogen.

Last weekend I decided to take a spare patch from my box and cut it into 3 (to make x3 @25) and stick it on with an additional 75 patch to make 100. A few hours after this my aches literally melted away, gone. I’ve had good sleep all week but that could have been down to my progesterone, and I’ve had zero hot flashes. I don’t want to ring my doctor and tell her I’ve been cutting my patches and giving myself a higher dose as it hasn’t been prescribed to me but I want the higher patch. Shall I ring and say I want to speak to someone about increasing due to symptoms? I don’t know what it is I think I always have this fear of being dismissed as I was told constantly that I was too young for peri when I first went to complain about my symptoms and to ask for HRT, it took me a few times for them to listen  ???

I can't see why they would have a problem with increasing your patch to 100mcg either way.

Personally I always try the thing I want out first (if I can access it) and present it as a fait accompli.

That way you bypass any "I don't think that would work", because you have already established that it does.

I would tell your GP about your returning symptoms and ask for an increase. Usually they're happy to prescribe up to 100 mcg as this is within the NHS guidelines. It's when you need more than 100 you hit the 'system says no' wall unless you have a particularly enlightened GP.

Thank you both. I will do that. I had a feeling they’d say it was too much too young or something but I’ve been reading online that younger women (I’m 38) usually need a higher dose of oestrogen anyway compared to older women due to the fluctuations in peri.

I have been aware of many posts advising changes in dose of estrogen. There are many factors to consider before increasing the dose, including practical aspects such as if using patches, are they staying on ok or causing skin irritation, which can affect absorption. Where are you applying the patches?--buttocks are believed to be the site for best absorption. Are symptoms purely hormone related, or are other factors contributing including nutritional factors, stress and lifestyle. Has a daily tablet been considered as an option, which suits many women well with minimal risk. Any possible change in regimen should be discussed with your doctor or nurse.
If after all this is considered and it is agreed that an increase in dose of estrogen is needed, then the progestogen used should be also increased proportionately. There is less chance of abnormal bleeding with HRT, which causes concerns and often uncomfortable investigations, when using HRT preparations that combine both estrogen and progestogen proportionately, eg a combined patch or combined tablet, rather than taking them separately. Please take all this into consideration.

My patches do stay on now as i put the hairdryer on it for ten seconds after applying which seems to stick for the whole 3/4 days. I’ve also started sticking them in my bum rather than thighs to see if that makes a difference. All I know is for that week and a half of cutting the patches to make extra on top of my 75 I felt great. No aches and pains, minimal itchy skin, even the skin under my arms that I’d previously scratched at wasn’t itchy at all and had healed, no anxiety and better sleep. Yet as soon as I went back down to my 75 patch I’ve had really painful wrists, back pain and shoulder pain, itchy skin again and red raw armpits where I’ve scratched and scratched. Also anxiety has came back and I feel chewed all the time especially on a morning. I’ve rang the doctors today and got an appointment for a HRT review on 13/8 so hopefully I can get some help x
