Adenomyosis and HRT

Hi, I’m new and 48, just been diagnosed with adenomyosis. I’m currently on 100mg estraderm patch and 1 utrogestan continuously, I started having peri symptoms mid 30s. I already have osteoporosis and scoliosis.  My oestrogen bloods shown 295nmol just before my period. My GP wants me to either have the Mirena coil or 2 utrogestan but every time I take 2 utrogestan continuously I end up bleeding every 2 weeks. Hence I don’t want to coil and I don’t want to move to 2 as I feel awful. I’m now aneamic and low ferritin so have been given iron tablets. I don’t know what level my oestrogen needs to be to improve my bone health but now my GP has said I either need to take 2 utrogestan continuously or she won’t prescribe me HRT due to the womb. Anger risk. I have had 1 appointment with a Louise newson clinic who think I am a poor absorber as I feel awful, but I have another consult on Thursday. Any suggestions or if anyone is similar what did you do. I am now bleeding every 16 days and for 10 days at a time Which my Gp says is the adenomyosis.  I don’t really want a hysterectomy as it’s major surgery and I need my core strength for my scoliosis. I’m stuck in the middle of a crappy decision. If I come off hrt I am horrible, rage, violent, sweats etc and I need to work.

Who is overseeing your bone health on a regular basis ?  Hopefully some1 will be along with advice. 

What did the Newson Clinic advise initially?

I had the bleeding every two weeks on continuous progesterone 200 mg, and eventually that turned into bleeding all the time and serious anemia problems.

I did get the mirena coil, and I really didn't want it because progesterone helps me with sleep, but so far it has been going a lot better than I thought it would, it just took some time -- they advised at least 3-4 weeks for it to "settle".  The mirena has basically stopped the bleeding after a month, just a little spotting.

I didn't think adenomyosis was diagnosed just from bleeding, but thought it was more related to pain; a couple doctors did tell me I had adenomyosis, more based on pain sx than bleeding, although I had that bleeding problems too.    If you have significant pain and not just bleeding, what I eventually found out from a provider was the fluctuations that can happen with estrogen patches can lead to both bleeding and pain with adenomyosis.  My doctor switched me to evamist (spray) and I feel better.

Hope something here is helpful.  I was worried I was going to need a hysterectomy too, when the bleeding didn't stop, and I'm glad I tried the mirena although I was reluctant.   I am curious how things would have been if I had started the evamist with the progesterone and if I wouldn't have needed the switch to mirena.

I do know some doctors in the U.S. think it is okay to do the .1 patch with 100 mg progesterone continuously, especially if you get regular scans.  My doctor said it was just hard to know for sure the 100 mg was enough, but for some women it is.

So I was diagnosed with adenomyosis by ultrasound. We only found out as the Lnewson dr wanted a scan done to check my womb lining thickness which came back at 11 and that seems fine, no fibroids or cysts etc but just adenomyosis. Re my bone health I am on high calcium diet and I and gp monitor it with dexa every 2 years. It’s because I had primary hyperparathyroidism for 15 years but I have had the malignant parathyroid removed but it’s left me with osteoporosis which is worse in my spine.
The plan with the LN dr was start me on 100 patches as I been on before rather than 6 pumps gel I was on then bloods after 6-8 weeks and thrn increase once we see what my levels were via blood. But this was before we knew I had adenomyosis. Now the gp is like well you have to come off HRT which a a no for me as I can’t function. I have another app with LN dr on Thursday but my gp is say 295 is high enough oestrogen level to control symptoms snd improve bone health

I was on micrgynon for 25 years up until 43 and I was really good on this but had to come off it due to my age. I don’t smoke, my blood pressure is very good, I’m not at all overweight snd I’m fit and healthy with no dvt risk. I don’t know if I can go back on this for adenomyosis

Morning Girls - I would take the advice from the menopause clinic - we are after all individuals and what is the GP basing his idea of 295 being 'enough'?

Just wanted to update -- I got a definitive diagnosis for Adenomyosis by MRI, based on a junctional zone measurement of 14mm.

I asked my doctor and she said she doesn't think HRT is a problem, especially with the Mirena IUD I just got.  She may have said something different if my adenomyosis was more severe, but just wanted you to know my doctor didn't seem concerned.
