HRT dosing question
«

Hi there, 10 weeks ago my GP switched me from Evorel Conte twice a week to Evorel 50 applied twice per week plus 200 mg micronised progesterone every night. I also use Vagifem.
The reason for the switch was the bleeding I kept getting on the Conte patches. The bleeding initially settled on the new regime but then a couple of weeks ago my boobs started to hurt and yesterday morning. I started a period. I’m assuming that 10 weeks after a change in HRT is still within the normal timeframe for bleeding episodes as a side effect but I was wondering if the progesterone dosage is normal or if it is too high? Just wondering if anybody else has been prescribed a similar regime. Thanks in advance all.

changes can be scary, even when we want them

Could it be that your own hormones are over-riding the HRT or that the regime is re-awakening them?
How often R U using the VA treatment?

The usual dose of progesterone in a continuous regime with a 50mcg patch is 100mg.
However your doctor may have increased this for you due to bleeding and possibly endometrial thickness if you had a scan, so you shouldn't alter this without discussion.
How old are you and do you know when your mother's periods stopped?
You may wish to consider a cyclical or hybrid 25/28 day regime until you are fully postmenopausal. This won't necessarily get rid of bleeding but it might make it more predictable.
Alternative bleed free options would be a mirena IUS, a synthetic progestin or if under 50 a combined pill taken continuously.

Thanks for commenting both. I use Vagifem twice per week. I am post menopausal, 57 years old, periods stopped at 54/55. Didn’t know my hormones could be re-woken lol! I’m pretty sure the bleed is hormone related due to the sore boobs beforehand, just hope this is a settling in phase and it will all get better again soon.

Although the nhs considers you meno after a year without a bleed some people do go longer than this and then have periods again, either one or several. Given the sore boobs it could well be your own hormones so I'd give it a bit longer and see what happens. If it's your own hormones utro is rarely strong enough to stop it.
