Spotting
« on: July 03, 2025, 07:39:45 AM »

Hi I’ve been on HRT for 4 years and still haven’t found the right balance
Everol 75 & now 2 x 100mg Utrogestan

After seeing my menopause Dr in Feb she advised that I try and reduce Utrogestan from 2x100mg to 100mg as it made me groggy
I was taking them orally , I’m post meno

However a few months down the line and I was Experiencing tummy cramps and spotting after using the loo not enough that I had to wear a pad it would seem that maybe I need the 200mg
On the advise of my meno Dr , I reduced the patch to 62.5 which is tricky cutting the patch , but the spotting has stopped almost immediately.

I’ve had a pelvic scan and a very small fibroid was found & thickening of 5.3
A gynaecologist is phoning me on Tuesday , I thought it would be a hospital appointment

What should I expect I’m worried.
thanks

100mg was probably not enough to balance the amount of oestrogen for you.  For some women, 75mg patch will warrant the 200mg continuous progesterone.  Had you tried taking the utrogestan vaginally - or one orally and one vaginally?  If not, it might be worth trying that with 75mg patch and hopefully you won't have the groggy effects of taking both utrogestan tablets orally.

Fibroids can cause bleeding but the lining is thicker than they like too so they may think you need more progestogen anyway. It's worth trying utro vaginally if you don't already but I found I still got groggy. Synthetic progestogen are more effective than utro so you could consider them, or else a mirena. I would stick to the oestrogen dose that controls your symptoms and adjust the progestogen to stop the bleeding rather than cutting down oestrogen to do it.

Thankyou for your replies so sorry for my late one .   All was going well cutting the patch to 62.5mcg but then today slight spotting again I’m so tired of it all I’m currently taking 2x 100mcg Utrogestan every night . Apparently my fibroid is very small but I agree it could be the cause

I’ve got an appointment for a pelvic scan again it was 7th Aug but I’m at a wedding so had to reschedule and that’s not until Oct .  I am worried I’ll have to phone my Dr again in the morning for advice
