Why have my levels dropped?

Hello,
I’m more a lurker than a poster but I’m just at the end of my rope today. I’ll try to be as concise as possible, I’m even boring myself.
I had a total hysterectomy in Jan ‘22.
I was put on Sandrena 1mg immediately-did absolutely nothing for me.
After months of to-ing and fro-ing to my GP, I ended up going to a menopause specialist as there was only so much my GP could do. They tried sprays, gels, patches…I simply couldn’t absorb them.
Fast forward to the specialist, he put me on Elleste Solo. Within weeks my estrodial had  shot up to 799 and I felt great. I was also put on Utrogestan.
My GP then got concerned it’s an oral HRT and said I needed to try another gel! So I did what she thought was best and changed again! Within a couple of months my levels crashed to 84. I changed my GP and they put me back on Elleste.
In August last year my levels were 700 again.
I had blood tests recently as I’ve not being feeling right and they’re now 344.
Why would that happen??
My joints are aching, I’m sweating buckets overnight, I’m a ball of anxiety and the mood swings are driving me insane.
Is the Elleste maybe not working as well as it was? Where on earth do I go from here??
I’m so sick of all this. Had I known the rollercoaster surgical menopause would be, I truly think I’d have refused the op. It’s just been hell.
Just for info, I has the procedure because if adenomyosis, endometriosis and fibroids.

I’m really hoping someone can shed some light, even if it’s a case of going to a new specialist. The one I previously went to said it’s still a good level at 344.

I'm sorry you've had such a rough ride.

Whilst 344pmol/L doesn't suck, and importantly is sufficient for bone protection and probably cardiovascular health, some women do need more than this to feel well.

This is especially likely to be the case if you are used to higher levels.

There is some variability in plasma levels depending on when you took your last dose, so if you were feeling well and had a blood test in the morning soon after taking your pill, and another on an afternoon, and got those numbers, I wouldn't see that as cause for concern.

However the return of symptoms in the context of lower blood levels suggests either reduced absorption or some change in how you are metabolising the estrogen.

Has anything else changed over the same time period such as other medications, diet, exercise, stress, weight gain/loss etc?

It may be that when you started the estrogen you had a low SHBG, but now this has come up so there is less "free" hormone.
