Hi, another query about breakthrough bleeding.
I’ve been on Evorel 100 for a few years and it’s been great.
I’ve switched from cyclical to continuous Utrogestan at my GP’s suggestion now I’m 54, almost 55.
I’ve been on this regime for about 3 months now.
Every fortnight I have a small bleed with cramps. In the last week I’ve had two morning light bleeds, 8 days apart. By lunchtime it stops completely.


I’m fed up. I don’t want a coil as my cervix is flat after loop diathermy and I think it would be painful.

I’m up to date with cervical screening.

Shall I go another 3 months to see if it all settles down?
I can go for days either no spotting.

If you were good on a sequi regime I would go back to that. It sounds as though you aren't meno yet and utro is rarely strong enough to prevent your own cycle. Oral progestogens tend to be stronger so you could try norethisterone if you want to be bleed free although a mirena is usually the best option to stop bleeding. IMO the nhs are far too quick to put you on a conti regime when your body isn't ready for it, there seems to be a manual somewhere that says everyone is meno by 54 when in fact some are in their early 60s.

I was on sequential and would always start bleeding before id finished 14 days of Utrogestan, but thinking about it, I didn’t spot at all other than this withdrawal bleed.

So, I think I’ll try going back to that!

It's the withdrawal of progesterone that triggers the bleed so it's likely the bleeding was your own cycle not utro. There's usually 2-3 days between the end of utro and the start of the bleed. If it happens again you could start utro a few days earlier so it stays in sync with your own cycle.

There are also multiple other oral progestins that you could take which can have you bleed free.

Whilst micronised progesterone does have some positives it is the least potent at endometrial control.

You could always switch back at a later point when likely to be fully postmenopausal.

Hi Hellyt, i have been on evorel 75 for a few years along with cyclic utrogestan, and about 8 montgs ago i asked to try a continuous regime. On cyclic i would start bleeding on about day 9 of utro, lightly for about 3-4 days. On continuous it seemed like you, every two weeks and no control. I decided 6 weeks ago to go back to cyclic as id rather know when itll happen. Im now on day 8 of my second month of utro and havent bled all month sobi think this is where i beed to be right now. I tried bringing the utro earlier but i still had my period on day 9 so im just going to go with it!

I had a similar problem and are now on continuous 200mg utrogestan and this has solved the bleeding issue. Unfortunately this also gave me chronic bloating to the point I was struggling to eat anything without pain (about 6 weeks)  I then switched to 100mg orally and 100mg vaginally. Which has completely solved that problem. Phew 😁. It seems to be working very well if I’m honest. I have been advised to do this for a while and can then try and do every other day vaginally or try and cut down to two sprays of lenzetto instead of the current 3. I personally feel it’s still my hormones overriding the hrt. When on cyclical my periods just did their thing completely out of sequence to the 200mg mg 2 weeks on and off. At the moment I would rather not bleed at all.
