Internal burning.

Hi everyone

Been awhile since I posted.

I have been on Imvexxy 2 x a week at 4mg. And started systemic estrogen .25mg 3 months ago and recently uppped to .5mg (3 weeks ago)

I use yes ob very often during the day.

The biggest issues I have is burning post pee or showering (even though it is just water) standing for too long or squatting or bending over. Even when I apply yes it can sting.

I also have a burn before I pee mostly when my bladder is full. Infections have been ruled out.

I am meeting with a nurse practioner who specializes in menopause on Friday to discuss all of this.

I wonder because Imvexxy is inserted higher up in the vagina it is not reaching the area that I apply yes too. Just inside the vagina walls when you open the lips.

I have tried Premarin and estragyn but my tissues do not like it.

Varying opinions where my gp upped all my doses (Imvexxy from 4 to 10mg and divigel and progesterone. But no guidance from there. I only upped divgel because that would be too much for the tissues all at once. Once they settle maybe upping Imvexxy.

But I am at a real loss. I still have an internal wound that is very slow to heal with loose skin around the edges (sign of healing).

I cannot tolerate yes water based as even yes ob can be stinging if I rub in too much.

My whole life has been on pause since this started in Oct. I am miserable most of the time and all I do is lie down a good portion of the day.

Gyneo did not want to up anything at all and was no help except a bioipsy. She did rule out Lichen based on seeing  keratization of tissues (white area that the skin has toughened up)

When I use yes ob it acts like a barrier but I still feel stinging milder but still there.

I realize that starting systemic estrogen can stir up the nerve endings and make them more sensitive. The tissues sort of look plumper.

 I am just so lost and have lost hope.
Would like to add a cream or increase the Imvexxy. I literally do not know what to do anymore.

Here in the UK Vagifem would be the equivalent of Imvexxy but Vagifem is only in a 10 mcg not a 4 mcg as well.

Personally I would use the Imvexxy at  10mcg that you have been offered as that is the localised oestrogen which may have the more direct effect on the tissues rather than systemic.

You probably do need a cream as well to rub on the area lower down that you notice burning. Use of Vagifem alone never reached my vulva area at what ever level I tried or for how long My problem is vulvar.

Have you got an estriol cream in Canada to try or only the Estragyn which is estrone ?

I use Aproderm a barrier cream when peeing on the vulva becomes a problem at times.

Have you reduced tea, coffee, spicy food ?

Yes I have reduced all those things to non existent. My diet is very clean.

I have this internal wound that is still healing which I suspect is the root of the post pee sting and showering. It is healing because I see loose skin around the area. Like a paper cut and often when that skin gets exposed by urine or water (no soap at all down there) or the yes OB shifts a little causes the burn.

I will ask the NP about what I should use cream wise.

I was prescribed 10 mg vagifem 5 days per week and use victory oil as a moisturiser. It’s been a game changer . I avoid baths and swimming and alcohol
