Does progesterone intolerance know improve over time?
« on: April 26, 2025, 12:57:46 PM »

Post menopausal, various HRT regimes all causing bleeding. Hysteroscopy ruled out problems, unable to have coil due to my weird anatomy (3rd attempt was by a gynaecologist who said it's not happening)

When I tried 200mg micronised progesterone it was massively sedating.  I'm trying 100g micronised progesterone with my combined patch but again feel tired/drowsy and tearful in the morning, improves in the afternoon.

My question is, is it worth persevering, has anyone found their tolerance improves? I stop taking it after a couple of days to check it is that causing the problem and it definitely is.

I tried a couple of days vaginally but it's generic micronised progesterone not Utrogestan and I can't find a definitive answer to whether it can be used vaginally or analy.

Thanks.

I don't know the answer but I have a similar reaction to utrogeston, it didn't improve over time and it had the same effect vaginally and rectally. Have you tried any other progestogen?

Some people do get used to it over time, but if you haven't tried it vaginally then that would be the obvious first thing to try.

Personally I would be comfortable using any generic micronised progesterone vaginally, however if you are only comfortable with utrogestan, then use that.

Otherwise there are tons of other progestin options including 150mcg desogestrel or slynd, and there is also tibolone as an alternative which often works well in progestogen intolerant women.

Oh dear! After Evorel & Utro I tried Evorel conti which has norethisterone but that wiped me out, now on Femseven which contains levonorgestrel which doesn't make me sleepy but does make my anxiety worse but the problem is they all make me bleed which the Drs don't like.

What have you tried?

I find I feel different every time I take it …. I am in Peri so everything’s always all over the place anyway but I do feel the Utro affects me differently every month. Sometimes I barely notice it and this morning I’m jolted awake at 5am in a heart racing panic … drives me mad.

I have a mirena now which suits me. Have you considered a sequi regime? They told me there's a slightly higher risk of endometrial cancer but agreed to it because I couldn't tolerate continuous utro (I used a 6 week cycle). At least with a scheduled bleed they don't need to panic. Tibolone might be worth considering. Cyclogest is sometimes tolerated better than utro but it's only licenced for ivf in the UK so your gp may not prescribe it. Provera might be worth a try too but I think it's a case of try everything and hope something suits you.

Hi Fianna

As with the others - my reaction to Utrogestan varies each month but always I feel fatigued and somewhat head foggy, always distrubed nighttime sleep due to bladder,, and sometimes a migraine.

I take it cyclically even though I'm in my 70's and I would agree with sheila on this point, if you can't tolerate continuous use and continue to bleed.

Regarding generic progesterone - I didn't know there was a generic one though Gepretix is a branded one almost identical to utrogestan.

I looked up information on this some while back so if this is what you're looking for I can find the thread. Gepretix can be used exactly like Utrogestan and can be used vaginally and should be experienced pretty much the same. It is not designed to be used anally/rectally. Cyclogest is the only such preparation  which can be used this way as well as vaginally.

It's just labelled micronised progesterone, no brand name but it is manufactured by Exeltis who I know also make Gepretix.  The capsules look different to Utrogestan as well
