Double progesterone or different pill

Hi
Last October I was bleeding continuously for  3 months 10 years post menopause
I went and saw a gynaecologist as my lining was thick and fortunately had a Hysteroscopy that showed no signs of cancer and the gynaecologist removed some small, polyps

I reduced by oestrogel to one pump a night and carried on with the utrogestan 1 pill

Just started bleeding  again, the gynaecologist told me to either double the progesterone or to try a different type of progesterone

Not sure what to do

Has anyone been in this position and what worked best for them
Even with my low does estogel, the progesterone isnt stopping the womb thickening

If you tolerate the micronised progesterone then I would double the dose rather than switch agents.

You may also find it works better taken vaginally as more is delivered to the uterus.

When you swallow a capsule most of it is converted into other metabolites without reaching the systemic circulation.

Thanks for that, so it's possible if I took it vaginally to start (normal dose) it might stop the bleeding anyway

I am only on 1 pump per night anyway

I would double it anyway as the lining needs to get thinner. They say more gets to the right place vaginally but no one really knows by how much (the manufacturer and the nhs think the dose should be the same regardless of delivery). I know you're on low oestrogen but the lining has still built up so perhaps you need more than others. Synthetic progestogens are more effective than utrogeston but utro is safer because it's body identical.

Hi gillybilly,
My GP changed me to conti regime about 2 months ago - I am on 1 pump gel & 1 utro which I take vaginally - I too am having a constant breakthrough  - it is mainly first thing in the morning ( pink & watery ) then dribble for the rest of the day then tails off in the evening - but enough to have to wear a panty liner 😕

I am also experiencing constant dull low stomach ache & headaches every morning and bloating .
I had been on 1 gel & 2 x utro  for days 14 -26 ( 12 days per month) then I had a bleed for 4-5 days - everything was fine . But GP said because of my age(64) having a bleed every month could lead to womb cancer - so I changed and have not felt great since .

I know I need to go back to GP - but she won’t let me go back to a sequi regime.

Any recommendations, anyone .

She isn't watching and the amount of utro you use is the same. I had the same argument also aged 64 because I don't get on with utro. They told me there's a slightly higher risk of endometrial cancer but they agreed to it. You may find you feel better on a different progestogen but if sequi utro suits you personally I'd stick to it.

I don’t want the bleed monthly but it’s better than constantly’leaking’
at least you know when it’s going to happen on sequi and for how long .

I need to go back to GP .

I agree with Sheila. You will already have a sufficient prescription to take your progesterone cyclically should you wish, as the overall quantity taken per 28 days is the same.

Cyclical does have a small increase in endometrial risk long term.

But you should have the autonomy to judge whether your quality of life is important enough to justify this small increase in risk.

This is the case with any medical treatment, however when it comes to women and hormones there is this very paternalistic attitude that women's quality of life is of such low value that it doesn't warrant tolerating any degree of risk.

Additionally, cyclical hormone therapy confers a lower risk of breast cancer than when the progesterone is used continuously.
