Heavy bleeding, any recommendations ? 😞
« on: April 06, 2025, 06:50:37 PM »

So, I came off HRT a couple of years ago, and the last 6 months or so my monthly bleeding is just awful, bleed through pads, clothes, bedding. Dr put me on desogestrel progesterone pill hoping it would stop my cycle, but it hasn't and my heavy cycles just continue with added spotting throughout the cycle as well 😥
Has anyone found a magic cure ? I'm 52 yrs old and fed up of being fatigued all the time.

One 75mcg desogestrel pill wasn't enough to shut me down either, but you can double the dose and take 150mcg every day.

This is perfectly safe and is supported by both the FSRH and BMS.

I would be extremely surprised if you were still bleeding after a few weeks on this.

You could also add estradiol to this if required.

This may be preferable to an IUS in the first instance as it is immediately accessible, non invasive and can be stopped at any time should you wish.

Mirena coil changed my life.

I'm just scared that if desogestrel didn't work, the Mirena might not work for me either, then I've got the hassle of waiting for an appointment to have it removed 🤷

I'm finding I'm having side effects of not sleeping and I've gone super dry down below and itchy. I've heard it shuts down ovaries so no estrogen is produced. If I added estrogen in wouldn't I need to take a progesterone that is recommended ? I don't think desogestrel is used for the progesterone part of HRT ?

It can be, at double dose, off label.

I started a thread the other day about zoley pill, but the later part is chat about double desogestral. Take a look.

I'm pretty sure you must have estrogen if you're bleeding so much.  :o You could test to find out. GP unlikely to do so. But you can rest yourself.

I doubt my GP surgery would prescribe off label 🤷 they're pretty unhelpful at the best of times.

I have had my estrogen levels measured as I have a pituitary tumour, one was 60, the latest one was 462 and that was natural, no HRT. So it must go low and spike high. No wonder we suffer so much with menopause symptoms 😥

Ok. So were you on the mini pill when you had both tests done???

I ask because I felt cerazette control my cycle 2 years back and I tested and both tests E was very similar in the 200s. But last year I tried cerazette again and it did nadda and my bloods were in the 400s.

So some of the ladies are saying here, taking double would mean it'll give greater suppression and control. Rather than my own cycle pushing through.

I think it's worth a try. I'm going to try it before I even speak to my GP. I'm very comfortable with trying it as there is 150mg desogestral in the combined pills.

Is there a meno clinic your GP can ask for advice?

I tried the cerazette mini pill but it didn't help my bleeding problems.  I have the mirena coil now and also came off the oestrogen patch (oestrogen builds up the womb lining so can cause heavier bleeding).  Since coming off the patch bleeding has settled a lot.  I don't plan to go back on the oestrogen anytime soon as I figure I must be still producing enough of my own.  I'm also 52.  I was very reluctant to try the mirena and I put it off for years while suffering every month.  Of course now I wish I had agreed to it in my early 40's when it was first suggested - hindsight is a great thing!

Crispiechick, no I wasn't on any tablets, patches or any medication, that was just my peri-meno hormone fluctuations 🤦
When I asked the nurse why desogestrel wasn't working this time to stop my cycle, she said it was because I am in peri-menopause.
Went to see the HRT nurse today, she wants me to try double dose progesterone desogestrel, and has also booked me in for a Merina coil in 6 weeks. She's also given me a prescription for trans something acid to stop the bleeding, it's 3rd day of just soaking through pads every hour. I just rolled my pad up now and the blood was just dripping out, it's horrific. Bled through my clothes again 😞

Hi Grace, it's weird because Cerazette worked amazing for me years ago, but now it's not doing anything 😞
That's amazing Merina has worked for you. The HRT nurse has booked me in for one in 6 weeks time. If all that fails it's ablation as the lady resort.

I really hope it works for you.  I had light bleeding for a month after getting it in.  I still know when I have a period now, as I'll get spotting but they are getting further apart, thankfully.  I was also prescribed tranexemic acid while waiting on the coil and it did work to lighten the bleeding a bit for me.  It was called Cyklokapron.  I had to take it about 3 times a day on my heaviest days as far as I recall.

Brilliant that you've been allowed to double up on the Cerazette in the meantime. And maybe that'll work for you.

Did you have to ask to try that or did they offer?

I used to take cerazette fine for years. But last year I just go periods through it. Mine are not troublesome (it's the only thing I don't suffer from) but it was clearly not suppressing my cycle either. I was told it's just my age. It can no longer control things.

Fingers crossed for two. Very interested to hear how u get on.

You could do a blood test now and one when settled on two...that'd be interesting. X

Crispychick,  the HRT nurse offered me double dose desogestrel. I did ask if I could have Cerazette and not a generic make but she refused saying they are all the same 🤷
I'd be interested to see what my estrogen levels are on 2 desogestrel. The only thing I'm not looking forward to is insomnia on them, I struggled to sleep on one tablet, so dreading taking 2 🤦. See what happens. x

Thanks

Let me know how you get on.

I only use cerazette as I had a really bad time years ago when I was switched to cerelle. Unfortunately you seem to have to experience something bad before they accept you are brand sensitive. 🙄.
