Increase in HRT question
« on: March 26, 2025, 07:28:25 AM »

Hi everyone,

I started my increased HRT dose yesterday with the Evorel 75 patch. I put it on yesterday morning and went through the day as normal didn’t really feel much difference so far. However during the night I woke up in a sweat, the sheets were damp and my legs and back were wet. I was then awake for ages and couldn’t get back to sleep. This being one of the first symptoms I had with peri around a year ago before I went on HRT. When I woke up this morning I noticed my patch had scrunched up a bit on my bum where it had been stuck. It was still on there but part of it had came away from my skin and I had to stick it back down properly.

Could this be why I woke up sweating because it wasn’t stuck down properly or wouldn’t it have that effect that quickly? I’m just worried now the dose is too high as I didn’t feel fully comfortable on the lower dose. Also when I google it says oestrogen dominance can cause symptoms, as I’m not at the progesterone part of my cycle yet.

Thanks in advance for any advice

If your patch had fully or partially detached this would explain the return of symptoms. This can happen pretty quickly.

I would suggest cleaning the skin with rubbing alcohol prior to applying the patch, using heat to bond it to your skin eg hairdryer or hand warmer, and applying a square of hypafix or softpore dressing over the top.

Also once a patch has come unstuck or bunched up, it is no good trying to reattach it, you need a fresh one.

The above advice comes from the infertility world - I used the patches for IVF so had to be meticulous about keeping them in place.

Also I would avoid seeking medical information from websites using the term "estrogen dominance" - whilst excess estrogen with or without inadequate progesterone can certainly cause problems this is not a medically recognised term.

Day 1 is obviously too early to tell, but something to bear in mind -

Having been through a chemical menopause for a month or so with no add back HRT, I found that with no hormones I had immense hot flushes. But, absolutely no sweats.

However, outside of chem meno I get sweats at times. So I can 100% conclude that night sweats are changing hormones, for me. Not low E. Whether they come with high E, I'm not sure.

In terms of estrogen dominance - It is certainly not a medically used term...in the UK. But other countries routinely prescribe progesterone only in early peri, as this is the hormone to decline first. It's mind boggling that it doesn't happen to British woman - but there we are.  ;)

So, basically, keep going but keep an open mind and monitor your symptoms.

I don't think most specialists in the UK would hesitate to prescribe a perimenopausal woman progesterone alone where it was therapeutically indicated.

Thanks for both your replies. I’m due to change my patch Friday so I have in the meantime put an adhesive waterproof dressing over the top of this one just to keep it in place. I will keep going and monitor my symptoms, it may just be that the increase in hormones hasn’t balanced out in my system yet hence the sweating. It has also been mild weather the past couple of days so I could have just been warm in bed. Who knows.

I remember when I first started HRT I felt sick and a bit dizzy the first few days but that settled and didn’t come back. I put that down to my body reacting to the hormones  ::)
