I am 52 years old and started my HRT journey in November 2023 and it has been a hot mess. The ‘only’ relief I have gotten so far is lack of night sweats, bone aches and hot flushes. My insomnia has gone from bad to worse. I am on Gynocadin gel and 300 mg Progesterone daily. Recently I started getting palpitations that scared me. I have been bleeding monthly since starting HRT and each time I bleed, my doctor’s advice is to stop hrt for a week and then resume. He has tried adjusting the dosage but nothing is working. I am so sad and desperate. Anyone on the same boat?

How much knowledge does your GP have about HRT?  Some regimes induce a bleed. 

Palpitations are common. 

As oestrogen levels drop muscles may become lax = aches and pains plus the body may become dry: inside and out.  Browse round, particularly the vaginal atrophy threads; make notes ;-)

As for not getting a response earlier, it will depend on where in the World a person happens to be, whether it's a weekend or Bank Holiday etc..  Hopefully some1 will be along with a reply, otherwise you can put the product names into the search box: individually: to read any threads that pop up.

What we're your periods doing before you started hrt? If you weren't menopausal this won't stop them. Have you tried using a cycle instead of continuous? Some progestogens can make your sleep worse, it might be interesting to see if you're any different when you're not on it

There are multiple different options and it is common for treatment to need a few changes before you settle on something that suits.

There are also a number of bleed free options which are well worth considering, especially as micronised progesterone has done nothing for your sleep.

If I were you I would try and get an appointment with a menopause specialist - whilst most GPs can prescribe bog standard hormone therapy they often lack the time and knowledge to provide the more individualised care that many women need.

Were you one year without a period before you started hrt? If not you should be on a sequi regime where you take progesterone for 12-14 days and have a monthly bleed. Continuous progesterone won't stop your periods if you still have a cycle of your own. For some people some progestogens cause insomnia (I'm one) so it might be worth a couple of cycles of sequi to see how you are on oestrogen only. Sore breasts are usually too much oestrogen or too fast an increase. They usually settle though you may prefer to reduce oestrogen and increase more slowly.

I was one year period free before I started HRT and my lab tests confirmed menopause. My hormone levels were zero or close to zero. I started with the lowest dose possible; 1 pump Estradiol  and 100 mg progesterone- but I bled anyway. This month was bad. I had severe cramps that needed medication and I was bleeding clots. I am currently on 1-2  Estradiol alternately and 300 mg Progesterone daily( which I have temporarily stopped). I am working with an Endocrinologist but I am considering seeking a second opinion. I desperately need sleep!

Hi,

the coil is another option . Get advice from a menopause specialist , who is trained in HRT. Have a look at the decision tree at the top of the page , you will find it under one of the tabs. Also try magnesium at night. Liz Earle has some good U tube video's. Meditation / breathing exercises can help.
