Guidelines say progesterone dose should be increased proportionate to estrogen dose.
Common sense says progesterone dose should be proportionate to amount of estrogen actually getting into the body. I know this is the opinion of Louise Newson and it is one I share.
So if somebody was on half a pump and had an estradiol level of 250pmol/L they would only be on 100mg continuously or 200mg cyclically. Then there is you on 4 pumps who also has an estradiol level of 250pmol/L plus clinical evidence of hypoestrogenism.
If that were me, I would take the lower dose of progesterone. You can get a private scan yearly to be on the safe side, in fact I believe this is a good idea generally.
Also "higher dose" progesterone is 300mg cyclically, not 600mg cyclically. So if you are taking 3 x 200mg it's no wonder you feel like crap unless that's a typo.
You could also consider taking it continuously unless there is a good reason for being on cyclical - if you wanted to play it safe at least that would only be 200mg every day.
