Vaginal Atrophy
« on: January 28, 2025, 06:39:13 PM »

Oh my goodness, I honestly do not think I can cary on with the burning, itching and throbbing pain in my vagina.

It feels like I have a broom handle covered in course grain sandpaper rubbing on it. It feels uncomfortable to pee and on a bad day sitting isn't pleasant.

Sex is totally off which is so sad because I recently re married and always enjoyed that part of our relationship.

Having suffered for weeks I saw GP who took a peek and said VA. I have been prescribed Estriol Cream. I cannot even think about inserting the cream at present as I feel so sore and sensitive.

I was going to apply the cream with my finger around the outer areas and urethra initially as this is where I am feeling the most pain. Then after a week or so try inserting.

Please can someone reassure me and firstly let me know these strange feelings like my vagina has been set fire too and like a golf ball is pressing on it are a recognised and horrendous part of the menopause and that my plan with the Estriol will work.

I was going to use it at night. In the day I have been applying vagisil dry skin gel after each time I pee.

Any suggestions would be very very welcomed.

I would truly appreciate some wise words and relief from this nightmare.

There are many, many threads on here on this subject so you are most definitely not alone. I too had the sandpaper broom handle feeling. Agony.

Your plan with the Estriol is a good start. Don't be frightened of the applicator, even though the thought of inserting that in there is terrifying, just smear some Estriol on the outside of it to ease insertion.

Have a good read at the other threads. Don't suffer in silence 🙂

Use the correct VA treatment instead of vagisel - loss of oestrogen is causing your symptoms.

Using on the tip of the finger whilst relax can B a starting point.  A small amount at least 3 times a day to keep the vagina softer and this should help with any dryness and itching.  When I need 2 I use KY Jelly which is water based, 4 me it keeps the vulva comfortable.

If the golf ball symptom continues after 2-3 months of VA treatment, do see a physio interested in womens health to check for a prolapse which can press on the bladder.  Replacing oestrogen will help too.

Start off slowly with a little on your ginger and as you become more confident push you finger a little higher when you are ready. I use the applicatior which comes with mine and I was tentative at first. It will take a little while  to get working but when  it does it's life changing.

Hi Primrose
So sorry your going through this
Atrophy is a monster
Everything your experiencing is typical of the condition and sadly it will take a while to get some relief The advice to get checked for a prolapse is excellent I had mine diagnosed at the same time as the atrophy I was in a terrible state and just didn’t know what was going on so much pain and discomfort day after day and knowing what was going on didn’t make it easier but did mean I could start getting things sorted If your lucky your gp will be knowledgeable and sympathetic but don’t be surprised if you struggle to get help there’s a massive amount of ignorance and apathy towards this condition
There’s a couple of really good facebook groups that offer fantastic support from other sufferers I wouldn’t have coped with all this without them I’ve had surgery for the prolapse now but the atrophy is ongoing because finding the right meds is often difficult  I was told to use Centraban after every wee to put a barrier on my skin which worked really well and have just changed to Vagifem pessaries Hope you get some relief soon

Hello Primrose,  I'm sorry to hear you're suffering with VA,  it really is awful because it's 'there' all the time. Apart from the advice re potential prolapse,  all I can add is that treatment will take time and estrodiol or an equivalent internally, with an initial loading dose daily for 2 weeks, plus cream daily externally is really the only way. The applicator for the pessaries is thinner than the one for the cream and if you use lubricant,  it is possible to get them inside without pain, even if you are on fire. Your symptoms being so bad, you do need to get a plenty of treatment inside too. Even then, you may find after 2 weeks, your symptoms are not under control enough and you need daily treatment permanently. Many of us on here do.
I use coconut oil as a moisturiser after each time I pee or bathe too. Also it may be you'll benefit from testosterone as that can really help the urethra from being so irritated if estradiol isn't enough.
So it's good you've got treatment quickly but it may get tricky to get a high dose long term. If you look on the private lives section here at a thread called 'the burning club' there's plenty of advice and experiences.  That said, even at a higher maintenance dose, you can have flare ups. I don't want to alarm you but just want you to know it's very treatable but it's helpful to read as much as you can on the subject in case your GP is limited in knowledge.
I believe there is a move to get the information leaflet updated inside the packs of pessaries and cream to say that it's a safe treatment daily long term.
Wishing you all the best, but definitely have a good read on here.

Thank you so much to everyone for your help.

I have the pessaries which have a much thinner applicator so I will brave it and start these tomorrow.

I will continue with the cream also.

I have a face to face appointment with very sympathetic and knowledgeable lady GP on Tuesday. I will mention prolapse and I have also started a list of questions based on all your answers.

Thank you, honestly, I feel so miserable with it all the burning and itching and general dull ache of it all makes me weep but I have started on a journey of improvement so fingers crossed.

I really appreciate the support xxx  :)

Yes, you really need to get the pessaries up there and use 2 for maybe 3 nights, get as much treatment as quickly as possible.  Don't wait because the longer you do, the harder it will be to get things under control and feeling better. You really need to bombard your lady bits with estrogen. Plus, and forgive my ignorance,  I assume if you might have a prolapse, you would initially need an internal exam? Well that is going to be so painful for you otherwise.

I can vouch for the pessaries.  I feel pretty lucky that I didn't really realise I had a problem with VA but I mentioned to my Dr that I was up to pee at least 3 times every night & she suggested I give the pessaries a try.  8 nights in and I started to sleep right through, it's amazing how much difference it has made
