is it safe to use estriol 1mg cream every day long term
« on: January 27, 2025, 09:09:30 PM »

I use evrorel conti patches - 1/2 twice a week.
Post menopausal , 62, no flushes.
however have recurrent UTI, vaginal atrophy,.
I am in a new relationship since my husband left 3 years ago and basically every time i have sex i get uti symptoms.
I tried using the cream as prescribed but it is not enough to manage symptoms so now i use some every day.
I have tried loads of over the counter creams , pessaries etc but this works the best.

I am a bit worried about cancer as have had two episodes of bleeding - scan showed uterus lining was not abnormal. however the bleeding seems to be after sex so not sure if lining of vagina is just thin... GP has not suggested any follow up.
i so want to keep using the cream daily.
does anyone else do this?
thanks again

You can use VA treatment every night 4 as long as is necessary.  When symptoms niggle I put an 'extra' dose up there mid afternoon and go from every 5th night to 5 nights in a row and review.  It can be Trial and Error.

U will need to use a lot of lubricant i.e. KY Jelly B4 being intimit [sp].  1 can insert the VA treatment the following morning.

Congratulations on the new relationship - scary and fun.  It's known as honeymoon cystitis ;-).  Logically your bleeding is probably due to 'over use' in a new relationship hence my suggestion of KY.

Many years ago I had a polyp which bled on contact, i.e. after sex, my Gynae wanted to remove it.  Privately so I refused  ;D.  It disappeared at peri ;-).

Hi pebbles, I use ovestin twice a day and estradiol daily. Last night I started using 2 estradiol pessaries as I have a bad flare up and I read another member does the same. It's such a low dose of estrogen that it's completely safe.
With regards to bleeding, I've just had that after sex. That's never happened before but since it was brief and immediately after, I wasn't concerned. It also seems I end up with BV after sex too.
So pleased for you to be in a new relationship but a very difficult time if you have VA. Are you also using testosterone?
I found that whilst it really helps with libido ( I know other ladies on here have mixed results) it has helped my overactive bladder quite a lot. Another tip is local anaesthetic cream before sex. Stings like crazy when you apply it but that only lasts about 10 seconds, but then oh so good!! You can buy it online if your GP won't prescribe. When I saw a gynae,  she did an internal exam to see what my vaginal muscles were doing and told me I was tensing my muscles, I told her so you would you if you had that much pain! So I make a real effort not to tense up during sex and found that, combined with the local anaesthetic, made everything much easier. Sorry tmi, but hope that might help.

Some quotes from the BSSM (British Society For Sexual Medicine) Position Statement that you might find helpful and reassuring.

“Studies have shown that around 70% of women have symptoms of GSM, yet only 7% receive treatment.
Women should be offered an examination and their decision to accept it or not needs to be respected.
The clinical response to treatment with topical oestrogen is usually rapid and sustained.
Women should also be advised that the information packaged with vaginal hormone preparations is out of date and factually incorrect. This needs to be changed by the MHRA.
The dose of vaginal oestrogen is very low, for example, using 10mcg oestrogen pessaries regularly for one year is an equivalent dose to just one 1mg of oestradiol HRT tablet.
If symptoms do not improve with vaginal oestrogen, consider increasing the dose, changing preparation (and consider DHEA) or using an additional treatment such as systemic HRT or two local treatments.
The frequency of using these preparations can be increased in women who have persistent symptoms as the doses of these preparations are very low.
Vaginal oestrogens can often improve urinary symptoms, including urinary infections.
Some women use vaginal oestrogen preparations more frequently than the recommended dose to improve symptoms sufficiently, for example Vagifem alternate days instead of twice a week. It is safe to use the treatment in this way as the dose is very low.
Women should be reassured that it is safe for topical oestrogen to continue in the long term (medication should be on a repeat prescription). Women should continue to be reassured about the long- term safety of vaginal oestrogen preparations."
