Hi
I have been prescribed utrogestan until I get my mirena coil changed hopefully next month, I think it’s as a precaution as it’s been 5 years since I had my mirena.

I tried utrogestan before, and taken orally it made me very lethargic and flat.
I have read it’s effective and safe to use vaginally, but I also have an estring and use intrarosa nightly too.

Does anyone know if it’s ok to use utrogestan vaginally alongside other vaginal pessaries?

Absolutely!

Whilst it might get a bit crowded up there, it is not at all harmful.

I would insert the progesterone first in order to get it higher up closer to the uterus, then the intrarosa.

I would say though that if you are needing both estring and intrarosa, you might want to review your systemic treatment.

I’ve been on systemic hrt and vaginal treatments for 9 years.
Started with oestrogel, mirena and estring, three pumps oesttogrl are my sweet spot.
I also use Testogel.

I had a horrible few years with GSM/VA and at that time I paid privately for Mona Lisa laser treatment which helped.
Bladder frequency and urgency have been a nightmare for me too, and intrarosa was suggested alongside the estring to try and help my bladder more.
I now have bladder Botox which has helped, it’s not perfect, but it helped.

I have to wait until next month to get my mirena replaced, and due to it being 5 years since it was last replaced, the utrogestan was suggested as a precaution, I’m just worried it will upset  things, mood etc if I take it orally

Just to add, I’ve been prescribed 200mg utrogestan nightly so it will be two tablet

If your IUS is 5 years old and getting changed next month, I wouldn't worry if you don't tolerate the utrogestan.

Unofficially, many women went beyond 5 years during the pandemic who were using it for endometrial protection, and used no other progestogen.

Whilst unlicensed, some clinicians now believe it can safely be used for up to 7 years as a result of observing no harm in this population.

I don't think that anyone should do this without clinical oversight, just that there's no need to call an ambulance if it goes 5 years plus a couple of months.

Try the utrogestan vaginally fur a week and see how I feel, if I tolerate it then at least it’s another option open to me.
Dont suppose you know if it might affect my bladder? Either positively or negatively? X

It can affect the bladder, unfortunately. I also couldn't tolerate Utro orally but vaginally it caused further irritation to my already sore and irritable bladder. I know there are others on this forum who've been affected that way but I'm not sure that it is a common effect, so might be worth trying until you get your new coil. As bombsh3ll says though, you will be fine without it for a month or so.

Thank you, I  have received my appointment for coil replacement and ultra sound on 7th Feb, I’m thinking I won’t use any more utrogestan as concern it might cause more problems than necessary, potentially bleeding and/or lethargy and low mood

I suffer with health anxiety and all this 4 years or 5 years thing surrounding the length of time a mirena is effective when used as the progestogen part of HRT, makes me scared.

You could always get an ultrasound scan done after 4 years if you are concerned, to check the lining is still thin, maybe paying for it privately if your GP won't agree to it.
