Advice needed please!

Hi all,

I’m 49, had heavy flooding periods since about the age of 40 so at 47 had the Marina coil fitted.. I wasn’t one of the lucky ones who had zero periods.. I’ve had mirena for 15 months and still have periods, albeit not flooding, but last months bleeding and spotting last 21 days!
I’ve had a TV scan, coil is in place and all seems ok..
since Jan 2024  my peri symptoms have ramped up..I’ve had terrible anxiety, irritability, hair loss, occasional night sweats (I say occasional as mostly sleep with just a sheet now) weight gain and aches and pains in right shoulder and lower back, insomnia, low libido and UTI after UTI! ! I was prescribed oestrogen (everol 50).. I stuck one on about 4 months ago, felt great for a week and then the bleeding started.. tried the gel.. exactly the same thing happened.. so hrt specialist started me on more progesterone .. again the bleeding ramped up.. I’ve no idea what I’m supposed to take.. with the continuous bleeds with my mirena, does it mean I need oestrogen to balance it out?
I’ve just started another period.. only finished last one 2 weeks ago.. should I start oestrogen today on day 1 of cycle?
Anyone else had these issues?
I’m guessing if I didn’t have a coil I’d be heavily bleeding the whole time so thanking my stars for that.. but the constant on off bleeding is just as draining in a way!
Just to add, had a blood test in 2022 and my oestrogen was 1348!!! So very high! But 2/3 years later.. could it be very low?

Morning.  R your periods 'regular'?

The UTIs - was a urine sample sent to a Lab for checking and what treatment was suggested?  This could be vaginal atrophy which your GP should have recognised .

Do U mean that you have used 1 patch  :-\  ???

Hopefully some1 on a regime will be along with advice.

Hi, That sort of endless bleeding is incredibly draining, I hope you are able to get some extra rest and iron while it's an ongoing problem?

There are many types of progesterone and they all have a slightly different effect on each of us, so I expect it's a matter of trial and error to find one that is right for your body and will work with your body to stop the bleeding.

As for oestrogen on sequi HRT. I'd take oestrogen from day 1 of any bleeding, even if the last cycle hadn't got far I'd start a new one because you are trying to sync your cycle to the HRT cycle and while doctors say HRT simply overrides our own cycle, it doesn't for so many women. Even four years post meno, confirmed by TVS, my cycle would kick in whenever given hormones to work with and my post meno cycle is still stronger than the HRT cycle. My bleed always started before the progesterone was finished, so at that point I'd happily ditch the progesterone and call it day one again.

The logic to that is the progesterone is only there to make sure we don't get an unhealthy uterine build up, so while bleeding, it's obviously not buildiing up at all and imo we don't need to poison ourselves further at that point.

I had very heavy bleeding, 200ml in three days was typical, so I tried;
the mirena coil, it stopped my bleeding eventually but caused low mood, out it came & I waited to be post meno for more choice, then
Evorel conti brought back periods that had stopped for over a year naturally, then
Provera made me want to die ASAP, I've never felt so awful physically or mentally, it had no effect on bleeding.
Utrogestan gave me the most painful heartburn/indigestion, no affect on my bleeding.
Cyclogest gave me the familiar instant low mood and made me bleed.
After that I refused to do any more experimenting with their obviously under-researched medicines.

All the above took a huge chunk of my life to test, I couldn't work while testing most of them because they made me so ill, it's not a small undertaking.

The thing that helped me most was Black Cohosh tablets, they are good for night sweats and anxiety imo. Now I'm postmeno it's not a problem, I can get tibolone and vagifem from my GP, they solve everything for me. Some doctors insist you wait to be postmeno for HRT, others don't, mine did. New research indicates starting earlier is better for our health so it might be for financial reasons that women are told to delay starting it.

It could be the mirena is helping and you need more of the same, or it could be, that isn't the right type of progesterone to stop you bleeding and you need something different.

I'm sorry not to offer more hope but you might just need to wait it out, most treatments come with bleeding for the first three months, so it's not easy to know which meds to stick it out with and what isn't so helpful.

Yes your estrogen could be lower now, and adding some estrogen may help with the menopausal symptoms you describe.

Regarding the bleeding, I know this sounds obvious but has anyone checked that the IUS is actually still there and in the correct position?

They can be expelled or migrate.

Another thing, if it is still present and correctly situated is to double check whether it is actually a mirena IUS that was inserted, not a copper T. That has happened.

Finally, if you are just unlucky and bleeding despite a mirena then you can add in another progestin.

There are multiple options but one of the most effective and easy to obtain is cerazette.

You could start off with one per day, if that doesn't stop it, you can increase this to two cerazette daily. This is supported by the BMS.
