Estrogen and testerone

At the age of 30 I had a full hysterectomy. I was put straight onto hrt evorel patches 100 and have been on them ever since. I have had a few issues along the way including stomach issues and anxiety but lately I have felt very low mood, anxiety fatigue and generally not myself. I have recently had hormones checked and estrogen is 137 and testosterone is 0.2 which I know is low. Does anyone else have these symptoms.

Hi there,
You’re right, your oestrogen levels are very low. Were you not offered a change of product when your results came back? It’s possible that you are not absorbing the patches.

I have been on gel before and estodot but didn’t find much relief from them. Any suggestions

I would get an appointment with a specialist to discuss your options, and don't be afraid of oral estradiol either.

Not everyone absorbs through the skin very well.

Hopefully this is a more recent problem than having had such a low estrogen level since the age of 30, but I would certainly push for a DEXA scan if you haven't had one.

Adding testosterone is also likely to help - this should have been offered anyway after a BSO at 30 but better late than never.

Thank you for your replies. I have certainly been feeling very low mood and anxious lately so maybe that is why. What should my levels be around does anyone know?

There are no official guidelines for target estradiol levels in women past the age of natural menopause, however a level of 250pmol/L is cited in NHS literature as the minimal threshold for bone protection. The late Professor Studd, whose work I am more inclined to trust, cited 300pmol/L for this.

Anecdotally, both professor Studd and Louise Newson have found in their clinical experience levels of 400-600pmol/L and occasionally higher can be required to treat mood issues.

I don't know your age but those with premature menopause including surgical as in your case, are strongly advised to maintain physiological estradiol levels at least until the average age of natural menopause. This could be approximately between 250 - 550pmol/L.

I personally know that I feel well around 400pmol/L, so I plan to maintain in this range, however we are all different in what we require to thrive.

Have you ever had a blood level done when you felt at your best? This can be a helpful guide.

I had a bone density scan about 6 months ago and all good.
