Endometrial thickening
« on: November 10, 2024, 02:30:55 PM »

Hi I’m post menopausal and on continuous hrt 25mg patch and 100mg Utrogestan

Changed to conti in June after being on sequi for 15 years had some v light bleeding when I changed to conti which I understand is expected for up to 6 months, bleeding stopped after 3 months and no bleeding since

Ultrasound revealed lining was 7mm and they’re referring me for a hysteroscopy

Is this really necessary I read that women with no bleeding that are post menopause the lining can be under 11mm before it’s an issue and needs further evaluation

From online -

Postmenopausal women without bleeding, no risk factors, and a global endometrial thickening of <11 mm do not require invasive investigations

It does seem to be something they do with anyone whose lining is thickened although mine was accompanied by spotting. Mine was 6mm and the outcome was all was OK.
I replied to you because I wondered if you'd been offered a general anaesthetic or had asked for one for the procedure? I don't want to alarm you in anyway but I do know that it's often not offered to you as an option and if, like me, you also have VA and haven't had children, it can be the better option. I just wanted you to have all the information you might need prior to having the procedure.

Hi Ebonybones,

I'm not sure why they are wanting to do a hysteroscopy, however I've just had a heavy bleed, (last one was last year) and I've just had a hysteroscopy.

I'm a real wimp when it comes to pain (even though I've had 2 children!) and the hysteroscopy was fine. I initially wanted it done under GA however she explained that it would be a long wait so decided to have it done without GA however I was given Penthrox for pain relief which I inhaled and it was all fine.

Hope any of that helps x

The difference is whether there is bleeding or not.

I am guessing you had the ultrasound due to bleeding, and now protocol is being followed re the results.

The best thing would have been not to scan unless and until it had gone on for more than 6 months, except if you have other risk factors such as obesity or previous anovulatory disorders eg PCOS, in which case your GP would have been erring on the side of caution.

Being on sequi for 15 years is also classed as a risk factor so this may have had a bearing on the referral too.

Now it will be like trying to turn the titanic around.

25mcg patch is a very low dose, and you could try to negotiate a repeat scan in 3-6 months, but I think they will probably insist on the hysteroscopy if you want to keep your HRT.

Hi yes I’ve been offered a GA, there’s no way I’d have one without  ;D

Hi yes the dr sent me for an ultrasound for v light bleeding even though it’s not been 6 months since starting Conti

I have no other risk factors so think the drs jumped the gun here

They’ve said I can stay on 25mg for now ( I was on 100mg when I was on sequi) I think I’m going to try and negotiate another scan in 3 months but if it’s above 4mm and under 11mm again with no bleeding they’ll prob insist on a biopsy again 🤷‍♀️ and I’m back to square one when I’ve read anything under 11mm in post menopause with no bleeding is normal

I’ve decided to go for the Hystercopy  :-\ even though I’m dreading it

As I’ve had pelvic pain since Saturday (I thought it was caused by the internal ultrasound I had on Friday but not so sure now)

I suppose a risk factor for me is my lining was 5.1mm when I had an ultrasound in July when I was bleeding from the change in hrt and now it’s 6-7mm with no bleeding, I’ve lowered my oestrogen from 50mg to 25mg too in June

And I’ve been on hrt for 15 years which is a risk factor
