I am so cross.

I was prescribe, gel + Progesterone at the beginning of September. My cycles are really erratic, between 27 - 85 days. The GP advised me to start HRT straight away.

So....the gel simply states "apply two pumps daily", the Progesterone reads, "take 1 daily on days 1-25".

I haven't had a day 1! So I started the gel on about day 25, and now I'm on day 75! So of course I didn't start the progesterone.

I've just contact the GP and they said, start taking the progesterone now, and call it day 1.

I'm so confused, the prescription notes were as clear as mud.

I have two questions

Should I be worried about the delay?

What happens when I don't have another period. If I take the progesterone for the next 25 days,  and I don't have a period, when do I start taking the progesterone again.

Taking unopposed estrogen starts to become a concern after about a year, that's when the NHS arrange a scan if someone has accidentally done that, not a month and a half, so don't worry about it.

Basically, the routine with cyclical HRT is to take the estrogen constantly and the progesterone two weeks on, two weeks off, and keep repeating this. It is designed to induce a bleed every time you stop the progesterone.

The thinking is that this will synchronize with your own cycle, however the gynaecologists who did the thinking were virtually all male, and whilst this works for some, it is often not what happens.

If your own periods are very infrequent you might be better just taking the progesterone daily than trying to dance to a tune that keeps changing.

Presumably you have been prescribed it this way because your own cycles are becoming infrequent. So you take 100mg utro for 25 days in every 28 day cycle. If you start today then today is day 1 and you take it for 25 days, stop for 3 days then start taking again for another 25 days (this will be day 1 of your next 28 day cycle). You should bleed 2-3 days after stopping utro. No need to worry if you don't, just continue with the cycle. It could be that there isn't enough of a build up to bleed every month or it could be your own hormones kicking in in which case you will bleed when they dictate.
The 2 weeks on 2 weeks off is a sequi regime for if you have a more regular cycle, then it's 2x100mg utro x 12 days (days 15-26).
