Always bleeding with HRT

I really don't know where to go next. I've gone from 50 patches to 75 as wasn't absorbing (175). Then on oestrogel. Tried 3 pumps (388) felt great. Reduced to 2. Everything has made me bleed. Been checked 3 times in 3 years.
Increased progesterone to 200 daily, that appears to have made bleeding worse.

Suggestions on what I do next. Main reason is for osteoporosis

Just to say I’m the same whatever hrt I’m on I bleed all the bleeding time no joke intended really gets to mr

A mirena is usually the most effective option for problem bleeding. Are you peri or meno? If peri you might be better with a sequi regime. Or you could increase to 300mg utro daily or change to a synthetic progestogen which can be more effective than utro.

A synthetic progestin would probably suit you better such as the mirena IUS if that's an option you'd consider.

If IUS isn't for you there are multiple oral options.

Synthetic progestins are highly effective against bleeding, often better tolerated and get an unfairly bad rap all over social media by proponents of "natural" progesterone.

Real world differences in outcomes between progestins and micronised progesterone are absolutely marginal, except for all the bleeding and increased endometrial hyperplasia.

Whenever anyone is evangelising about body identical progesterone, they completely omit the harm of women undergoing stressful and invasive investigation, often repeatedly, and many quitting treatment, because of bleeding problems that would not have occurred on a stronger agent.

Please don't feel like micronised progesterone is your only option when there are several alternatives.

I'm reading this post with interest as I'm in a similar position. Having had  bleeding previously I figured I don't get on with utrogestan and was always sold the premise that synthetic progestins are to be avoided like the plague.

Good to read your knowledge on the subject x

I switched to the mirena for this reason.  I still get periods but they are much lighter in comparison to what they were.  There are also further apart.  Utrogestan just wasn't strong enough for me.

Progestins do get a bad rap, I agree. I was trying to find out, how many more BC cases are found with a progestin vs progesterone.
Quitting is not an option as I have osteopenia, plus the other benefits of HRT for mood/anxiety/heart/brain/ less likely to get diabetes & bowel cancer.
Hopefully I will get a knowledgeable GP when I go on Friday
