Bleeding/change hrt

Hi everyone, newbie here.
I am 60 years old and post meno.
I was on femoston conti for 4 years then had a blood clot so put on everol conti. I had a blood test to see if I was absorbing, pmol was around 96. Then onto sandrena 1.5 and utrogestan 100 every night. I have bled every few weeks for the 9 months I have been on this.
I had a hysteroscopy 3 weeks ago, there was a polyp removed.
I had read about the 3 day break from utro to re set the receptors, my doctor agreed to my trying it which I did at the beginning of the month and today I have started to bleed.
I am wondering, as I don’t want to up the dose of utro as it can still make me a bit groggy, low mood, if I have to stop gel and utro then my only option is the patch again because of the blood clot, is it better to have low pmol than nothing?
I don’t want a coil due to a bad experience with it in the past where it moved.

I'm not I've understood this correctly. Are you stopping oestrogen as well as utrogestan for 3 days? Because you should continue to take oestrogen as normal, it's only the utrogestan that stops.

This isn't advice but my own personal story as I know that getting progesterone right can be difficult for some people. I have really struggled with continuous progesterone. I am 60 now and around the age of 56 I started taking Kliovance continuous but I had bad anxiety with it. So I changed back to having a bleed with Oestrogen and 200mg Progesterone 12 days. Then a year ago my doctor advised me to use continuous progesterone as it is recommended for older women. 100mg wasn't enough to stop bleeding and so I took 200 mg per day but I had a lot of problems with breakthrough bleeding, exhaustion, irritation below, dryness and generally just didn't feel good. I realised that I used to feel great on a sequential regime and decided to go back to that on a 6 week cycle. It's been two months now and I feel really good again with good energy levels. I have also recently asked my doctor to try testosterone and this has been great so far. . Obviously having a bleed again is a slight nuisance but in overall quality of life it is a small price to pay.

Each person is so different and I know there are plenty of people who do well with continuous progesterone or the coil. It may just be a case of finding the right progesterone. Don't be afraid to persevere until you find the right regime. It may even be worth paying for a consultation with a private menopause specialist for advice. You can still have your prescriptions and continue treatment through the NHS but it might just help get on the right track.

There are multiple other oral progestogen options that you could take alongside your Sandrena gel, either cyclical or continuous.

Options include
Desogestrel
Slynd
Norethisterone tablets - same as in the combination patch.
Medroxyprogesterone acetate

Using 2x combi patch (this is still only 100mcg estrogen)

Topping up the combi patch with a bit of gel - this is unlikely to be sanctioned by the NHS as theoretically the progestin may not be enough however this strategy is used by some private specialists with monitoring.
