Thickened endometrium

Hi, I've been on Sandrena and uterogestan for 3 years, I just had a private endo' scan and it's thickened to 11 mm. They've advised a GP appointment for follow up.

I've done some reading since and although I've been following the GP guidance on taking Uterogestan, days 15 - 25 of my cycle, (10 days a month), the advice may have been wrong. Everything I've read says 12/14 days.

Anyone got any advice/experience on this?

Hi have you had unscheduled bleeding or is your bleeding regular ?

No bleeding between periods, periods regular. (my post said my endo' was 9 cm! Actually 11 mm) I think it's supposed to be <8 mm.

Re your endometrium - whether or not it is classed as thickened in a problematic sense depends entirely when in your HRT cycle you had the scan done, and also whether you are peri or post-menopausal.

Ideally a scan shpuld be done immediately after stopping the progesterone and after the withdrawal bleed has finished, or rather when it is tailing off to spotting - as this is when it will be at its thinnest. However if you are peri-menopausal - say - early peri - and still have strong cycle then your own hormones may also contribute to the thickening of the endometrium.

I can't remember off hand what the limits are but certainly the limits before investigation are far greater for those on HRT than not and if you have had no unscheduled bleeding personally I would not be worried.

Why did you have a scan as a matter of interest? Days 15-25 inclusive is 11 days but maybe you only took it for 10? In any case that (10 days) is the minimum for any progestogen. The research studies showed more instances of endometrial thickening using this duration, hence 12 days was arrived at as the ideal (which may be too much for many women on low doses of oestrogen). With combi patches it's 14 but that's only because of  convenience not for any medical reason.

Depending on your oestrogen dose 12 days is the licensed dose for utrogestan to be taken cyclically.

By all means go for your GP appointment to (hopefully) put your mind at rest, but this may well involve invasive procedures such as hysteroscopy or biopsy, although they should offer a repeat scan in the first instance. If so make sure it is at the optimal time as above. I would not be at all alarmed right now!

I was going to say if you haven’t got unscheduled bleeding then there’s nothing to worry about .

Thank you for the reassurance. I have no idea if I'm peri or post anymore. I take HRT so I have a bleed every month, so who knows. I'm 51.
I've had absolutely no advice from GP, just resistence in past 3 years, same old c**p as most. That's why I had the scan, I've been self medicating with HRT, trying to find a balance in symptoms. I take 100mg  Uterogestan vaginally, so I wanted to know if I was doing OK on this dose, or slowly killing myself!

I did go to GP and they've referred me to a Gyni' clinic who will perform another scan tomorrow, which will be at the start of my bleed.

I won't worry, I'll see what they say and try to get some guidance from them about where I am on the peri-post timeline.
