Please Help with vaginal dryness
« on: August 22, 2024, 04:04:13 PM »

have some sleep issues causes by waking up at night to pee. Some anxiety and headaches before taking anything. I always had high energy levels and have not noticed a decrease at that. But the bigeest issues is the vaginal dryness.

I am also without a period for 120 days at this point. Not that I mind miss that part, but the same thing happened last summer. Disappeared and reappeared so who knows? I have had a lot of stress in the last two years which my dr said may have triggered all the symptoms maybe even menopause possibly not sure as it’s early still.

I am usually pretty active i run, do yoga, pilates etc. I was using Gynatrof to help relieve things. But spoke with my dr and she at first recommended estrogen and progesterone.

I waited and did some more research and spoke to her again and we decided on a prescription for vagifem.
My main symptoms that literally at times are crippling is the dryness.

The weather was slightly cooler the first few days of vagifem, i thought i was seeing an improvement. But i came back from a run this morning and its hot again. So i used  Gynatrof which has been taking the edge off the “razor blade feeling”.

I understand it takes awhile for it to kick in as i am in the loading phase but what can i use right now to help with this, or is what i am using okay. Not sure about interactions. I am trying to hydrate with electrolytes which tends to help. My dr is also on vacation for a week so that doesnt help

The last month has been the worst, i am also a hot yoga teacher so does not help that i am sweating all the time.i shower almost daily thinking it would help me.

The dryness has affected me doing yoga, running, working out, walking. I am so miserable

Use the 'vagifem' every night and a moistorisor i.e. 'sync' or 'yes' and your GP can prescribe 'ovestin' to slar round the out labia.

I use 'ovestin'/estriol regularly inside and out and KY Jelly outside when necessary.  If I get the need2P I swallow two 'nurofen' 3 times a day to ease the nip as the urine flow shuts off.

Browse round.  Make notes.

The heat makes the condition worse and its only the consistent use of topical oestrogen that will ease your symptoms in whichever form you find the best that suits you. At the beginning it can be trial and error but you will get there. Moisturisers have their place but they can't replace the hormone treatments that you need.

Quote's from the Position statement for management of GSM that I know will be very helpful to you

"The clinical response to treatment with topical oestrogen is usually rapid and sustained.
Women should also be advised that the information packaged with vaginal hormone preparations is out of date and factually incorrect. This needs to be changed by the MHRA.
The dose of vaginal oestrogen is very low, for example, using 10mcg oestrogen pessaries regularly for one year is an equivalent dose to just one 1mg of oestradiol HRT tablet.
If symptoms do not improve with vaginal oestrogen, consider increasing the dose, changing preparation (and consider DHEA) or using an additional treatment such as systemic HRT or two local treatments.
The frequency of using these preparations can be increased in women who have persistent symptoms as the doses of these preparations are very low.
Vaginal oestrogens can often improve urinary symptoms, including urinary infections.
Some women use vaginal oestrogen preparations more frequently than the recommended dose to improve symptoms sufficiently, for example Vagifem alternate days instead of twice a week. It is safe to use the treatment in this way as the dose is very low.
Women should be reassured that it is safe for topical oestrogen to continue in the long term (medication should be on a repeat prescription). Women should continue to be reassured about the long- term safety of vaginal oestrogen preparations."
