Advice / Experience with Cyclical HRT?
« on: August 03, 2024, 01:35:05 AM »

Hello!
I'm new here and I'm also still looking for my optimal path with hrt (started hrt 5 weeks ago). I had done lots & lots of reading and research prior and then asked my Doc for Estradiol transdermal gel & progesterone micronized capsules (utrogestan in the UK).

I'm in Canada and my Doc is a regular family physician in the public system, not a specialist.

From my research, I'd really like to try the cyclical method with my hrt. However, my prescription is for daily progesterone (100 mg) and 1.50 mg of Estradiol (0.75mg of gel, twice daily).

I can easily take the Progesterone starting only at day 15 @ 200mg instead of 100mg/ daily however, I'm not sure how best to manage the dosing of the Estradiol before & after the 1st 15 days...

My question: Would it be advisable & beneficial to increase Estradiol a bit in the 1st 15 days, then reduce it a bit when I start the Progesterone for the next 11 days?

Any advice and/or experience with cyclical dosing would be greatly appreciated! My Doc has never even heard of the cyclical method!

Cheers from Canada!

Hi C.Y!

I'm on cyclical hrt and my GP prescribed 2 pumps of oestrogel gel every day. The 200mg  of utrogestan I take only days 15-28. I haven't heard of anyone changing the oestrogen for part of the cycle as I think it's usually the same dose for your whole cycle. This approach seems to work ok but be aware your own cycle can sometimes get out of sync with the HRT causing unexpected bleeding. The cyclical/sequential method is commonly prescribed in the UK for those in perimenopause.  After my 4 month review my GP advised stick with cyclical for a year and then another review and start continuous which would be 100mg utrogestan / oestrogel daily.
Hope you find the HRT helpful - I'd recommend giving it 3 months as by then you should know if it's beneficial. Have a lovely weekend  :)

The estrogen is typically kept the same every day.

If you are taking the progesterone cyclically you should take 200mg ie two capsules daily for two weeks.

100mg is enough to protect the endometrium if taken daily but may not be adequate if only taken half the time.
