Sequential or continuous? WWYD?

I've posted a few times regarding HRT and had great advice so I'm back again.

I struggle with progesterone, it affects me badly mentally.

I've had failed attempts with Evorel Sequi which was great until I got to the Conti (progesterone part) where I had awful anxiety and panic.

I then had the Mirena which nearly sent me over the edge despite already having one up to a couple of months before this one was put in.

I've asked for Ustrogen to use either 3 monthly or vaginally but my GP won't do either without Gynae back up so I have asked to have the Ustrogen to take orally even though I intend to use it vaginally.

My confusion now is she is classing me as post menopausal as the only bleeding I've had over the last 6 years was when I had the original coil out and when I had the next one put in (mid April) nothing since. But surely the original coil was stopping my periods?

Anyway she has prescribed me continuous HRT so Evorel 50 patches and 100mg Ustrogen to be taken every night. Will this be okay even if I'm not post menopausal?

I hope this all made sense! This HRT stuff confuses the hell out of me 🤣

It will be absolutely fine.

The worst thing that can happen if you go onto combined earlier is a bit of unpredictable bleeding.

You didn't say your age but you are probably old enough to know your way around a pack of sanitary towels 😉

Regarding your dose, 50mcg estradiol is not very high and if you are taking the progesterone vaginally it delivers a higher concentration to the uterus, so some specialists are happy for the progesterone to be taken every other night vaginally.

If you wanted to take it cyclically, this would equate to none for 2 weeks then 1 every night for 2 weeks.

If you would be taking less than your prescribed dose without medical supervision however it would be sensible to book a private scan at 6 months or so to make sure your lining stays nice and thin.

Thank you for your reply, I’ve just turned 50. I’m so anxious about using progesterone after the last 2 reactions but I’m also fed up of my mood swings, forgetfulness and general lack of energy and motivation 🙁

At 50 you could be either. Perhaps try the conti and see how you are, some people can tolerate 100 but not 200. For me there isn't much difference so I'm staying on sequi to get more 'good time' (200mg x 12 days, days 15-26). The NHS and bms have always said it's the same dose orally and vaginally, it was Newson who said you could halve it but I think she's changed her mind now and people are being prescribed the full dose. Obviously the higher the oestrogen the more risky it is to take less progesterone and having regular scan is a good idea particularly as you don't know how much your own hormones are contributing.

I asked to go straight onto continuous, it can cause spotting but otherwise I was happy as it agrees with me and helps me sleep
