Every month my period starts on day 8 of taking utrogestan. Am I meant to carry on taking it until day 12? If I do I get really annoying cramping and on and off bleeding for 4 days until I stop taking it when I get a proper bleed.

I have tried trying to resync it with my cycle by taking it 15 days after the first bleeding but the same thing happens regardless. Any ideas? I would like to stop it on day 9 but don't know if this is advisable.

The official answer is that you carry on regardless. I didn't, given that I was bleeding already I didn't see the point. If you cycle is regularly less than 28 days you could start taking it earlier but you still need the 12 days. My natural cycle is longer than normal so I just started later.

Thanks Sheila, I thought that might be the case. It seems that whenever I take it in the month (either early or late) I still get a bleed on day 9, not sure if that means I don't have a natural cycle anymore? Do you have scan to check everything is OK if you take it for less than the recommended days?

My situation was a bit different. I had problems when I used a 28 day hrt cycle as my natural cycle was longer so it was forever out of sync. Bleeding was determined by my own cycle with an occasional extra hrt induced bleed. I get sore boobs on progesterone so I solved it by taking the 12 days of utro when they started hurting and this kept the 2 cycles in sync. It's normally the withdrawal of progesterone that triggers the bleed so I'm not sure why this is happening to you. Usually if you bleed early it's because your own progesterone stopped but bleeding on day 9 on different cycle length is a bit odd and I can't think of an explanation for it. As long as have a good bleed it is probably OK (not sure you can say definitely OK). If you do have a scan it needs to be at it's thinnest when you've just bled, if before a bleed it will be be thick anyway.

Ah ok thanks.  I have been doing a bit of googling and found this

In the section Management of abnormal bleeding during combined sequential hormone therapy it states
'In case of spotting before the 10th day of the progestogen sequence, the progestogen/estrogen ratio may be to high, and increasing the estrogen dose seems to be the most appropriate measure.8 If this measure is unsuccessful, a reassessment of the endometrium should be scheduled.'

Has anyone got any knowledge of this?  My GP does not seem to know, should I be asked to be referred to gynaecology for more specialist advice?
