A couple of days ago I tried to request a repeat prescription for Evorel Conti patches. It was refused twice without any explanation which left me having to guess. I hadn't requested any since last September and only have 2-3 weeks supply. With the current shortages, I didn't want to leave it too late.
I got a reply saying I had to fill in their HRT review form. The quality of their form is poor/inadequate. I had to tick boxes to say which menopause symptoms applied to me but there were only 6 possible options  (just basics like hot flushes, low mood etc ) and none were relevant to me. I had to say yes to another question (about side effects of HRT even though I have none) just to be allowed to add anything extra. There was no mention at all of VA, prevention of osteoporosis and muscles/joint aches so I added them to my form.

Do other members have to complete a similar form every year and how comprehensive/flexible are the questions?

I would phone the doctors surgery and hopefully the receptionist should be able to help/advise or point you in the right direction so that you are able to get your prescription
