How to cut Cyclogest pessaries without them falling apart?
« on: August 02, 2023, 04:47:09 PM »

Hello

I have been searching on the forum about cutting Cyclogest pessaries and have seen that some women do so. I am having a lot of difficulty cutting them without them crumbling! I can't seem to do a clean cut and it all ends up in a bit of a mess - I have been using a scapel. If anyone has successfully managed to do this please could you advise me on your technique!

Thank you very much in advance!

Hi Flossy - I started to trial Cyclogest 4 days ago as I finally discovered I cannot tolerate Utrogestan because of various side effects… I needed to find a way to cut the 200mg into quarters so I could start in 50mg doses. I use a Stanley knife that I disinfect in hot water before cutting them along the side into quarters. Cutting longitudinally did not work for me as they crumbled or I had less control over the amounts cut. It works ok so far, just cut slowly so as not risk them crumbling! x

hi flossy

i use cyclogest 200mg pessaries and i cut them in half so i use approx 100mg a day.

i bought a chopping board (that i obviously only use for this purpose)  a sharp knife and i have a tape measure and i cut them in half.

i havent noticed them crumbling however quartering them would be a problem. when i first started using them i tried to cut them into 50mg and it was no good as they crumbled however it is easier now im only cutting them in half.

also same as Furyan I dont cut them longitudally as that would never work

sorry i can't be of more help. It is all a faff though isnt it xxx

Hi Furyan and discogirl, thank you very much for your replies, apologies I have not been back on in a couple of days to respond, my cat has been very ill and I have been back and forth to the vet and not online much.

When you cut them width ways, how do you ensure it is half? As even with measuring, the pessary is a different shape at each end (pointed at top and narrows down at the bottom) so I don't know if the two halves would be equal? Or do you think it doesn't matter too much as if you do each half on subsequent nights it adds up to the one complete one anyway? That is what I have got stuck on and why I was trying to cut length ways, though as you say that is tricky and not v successful!

Thanks again, it is just one more complication on this whole complicated menopause journey!!

Hi Flossy
Some people use progesterone on alternate nights so assume it wouldn't matter if the dose varies a little from day to day. There won't be that much difference in cutting the cyclogest. Would they cut easier if kept in the fridge perhaps, although there may be a lowest temp you have to avoid, not sure?

Thanks penguin, that is what I thought re the dose, is not like estrogen where you want to keep in constant. Fridge might be an idea, I will try that. Alternatively I was thinking of heating the scapel so it kind of melts when it cuts. Seeing others here say they use a Stanley knife, maybe that would work better than a scapel.

I've also used a pill cutter for other tablets, particularly those that don't have  dividing line, so you could try that too. However not sure how you'd disinfect it and Cyclogest is like a hard cream type substance isn't it, so would need to be cleaned off.I remember it from IVF days.

Yea I'd just checked the storage instructions on the cyclogest though, just to make sure a fridge isn't too cold.

Hi Flossy,

When I used to cut Cyclogest in half I kept the wrapper on and had no issue with crumbling plus it provided a bit of cover for the unused bit. After cutting, I used to just snip the side of the wrapper and it would peel off.
I now use it on alternate nights so don’t cut anymore.

At least you have a few suggestions to try here. 😊

Hello ladies.

I cut a 200mg Cyclogest pessary in half and use on alternate nights. I keep the pessary in its wrapper and just use scissors to cut across the middle ( cutting along the length of the pessary would be too difficult I think).
There is some crumbling but not too much.
I don't keep them in the fridge as room temperature is okay and frankly I wouldn't want to insert an chilly pessary anyway lol.

Take care ladies.

Thank you Evie and Kathleen, good point re the fridge - inserting it cold might not be very nice! I have tried cutting it whilst in the wrapper and was a bit more successful but one half still crumbled. I might get a Stanley knife and see if that works better, as these are only available on a private prescription I can't really afford to keep wasting them when my attempts are unsuccessful!

Hi Flossy,

I do measure mine however as you say even with a tape measure, measurements can be out. The way I look at it is over 2 days I will have inserted 200mg, so as long as I put one half one day up and the other half the other day up, then over two days that will be 200mg.

If I think about it too much I would end up over thinking and I can't go there as my mind is in a bad enough state at the moment as it is.

Its not ideal but until pharmaceutical companies realise that the 'one size fits all approach' with some women doesn't work, its what we're stuck with x

Thanks Discogirl, yes that makes sense re the cutting, I don't think it really matters that the halves are not exactly the same, I am the same with overthinking so I totally understand where you are coming from!

It would be so much better if Collins or whoever manufacture them could make into 100mg pessaries however as they're off licence for hrt and used for fertility this must be the reason they're 200mg or 400mg.

And yes, my mind definately isn't my own anymore, it seems to go off on random wanderings!!!

So frustrating and as if we don't have enough to deal with xxx

I agree Disco, I did think about measuring at first but decoded not to faff with details to that extent since my body would have gotten the same amount over four days anyhow. As said, I am cutting mine into quarters to give 50mg initially, so it’s a bit trickier, but the Stanley knife cuts through without them crumbling so all good. Like you, I’m cautiously have initial thoughts about Cyclogest but we shall see… xx
