Spotting- new to vagifem

Hi
I'm in peri with reasonably regular periods and have been taking estrogel and utrogestan (vaginally days 15-26) since end of December. Always had a bleed  2-3 after stopping utrogestan. However this month I also started with vagifem, currently on day 7 of the loading dose. My last utrogestan was last night but I have had a bit of brown spotting today. Is it normal for the withdrawal bleed to start so soon? Or could it be the vagifem? I have been putting it in of a morning due to using the utrogestan at night and the vaginal walls have felt a bit burny / hot about an hour after inserting it, but not strong enough to stop. Any ideas please? I have health anxiety so any deviation from the norm scares me! Thank you x

If it's any comfort my period started four days after finishing utrogestan this month, last month it started on the last day of utrogestan and the month before that I was on provera and my period started on day nine of provera.

Periods are unpredictable in perimenopause, that is one of life's certainties  ;)

That is a comfort, thank you. I'd thought that my progrsterone level would have to drop over  period of time before I bleed but perhaps not. Perhaps this is my normal cycle underneath.

I have lower level tummy ache this pm too and my sore boots have gone, so perhaps this is actually just a slow start to withdrawal bleed.

I had a couple of blood staining when I first started vagifem. After about 3 months though. I don't have periods now. I was told to stop, had internal exam and ultrasound.  Everything was ok

It is unlikely to be due to the vagifem - mainly because, since you are already using systemic oestrogen, and also still having fairly regular natural periods, then your vaginal tissues are likely to be fairly well oestrogenised ie not fragile.

That being said, depends how you are inserting the applicator - if you've been poking it up quite firmly ( I sometimes just give it a quick shove because I'm in a hurry - I too insert mine in the morning - always when I change my patch) you might "hit" the vaginal walls and cause minor trauma which could result in spotting. I know I sometime poke mine in the wrong direction and definitely against the V walls - though it hasn't given me any spotting.

Other than that, as others said - could be your cycle breaking through it you are peri.

Spotting is much more likley to occur in post-meno women starting vagifem for the first time and who aren;t already on HRT, until the tissues have plumped up - probably the reason for your initial spotting Jules?

Brown spotting is old blood so your hormone levels R probably not 'enough' to empty the womb completely. When you say spotting how much? enough to fill a pad in the morning or rust like spots?

Thanks both. It turned out to be a proper period/ withdrawal bleed by this morning so all good and defo not the vagifem. Day 28 of both my hrt cycle and what would also be day 28 of my underlying normal cycle. Along with my usual period pains so feeling pretty rubbish but also relieved.

Hurdity, I use my finger to insert so don't think it could be that, but I will definitely be careful.l going forward.

I just hate the unpredictability and not knowing for certain part of all of this, it really plays into my anxiety.

CLKD, it was two separate bouts of rust like spots,  nowhere near enough to fill a panty liner and turned into a full on period/withdrawal bleed this morning. Perhaps it was just slow to start?

Looks like it!   I would have a heavy bleed for 10 days with intense, excruiating pains until day 3, a gap of 2 days then brown staining.  'normal' 4 me.

Feminax helped the pains the idea being to take them 2 days B4 a period is expected.

Oh I remember feminax, wonder if they still sell it, that used to work well for me. I'm using a paracetomol with codeine atm, only need them for first day. Early night and hot water bottle tonight and I'll be fine tomorrow!

I'm not sure, I was just told it can be a side effect of it. It wasn't very much anyway but first time I'd had it since my periods had ceased
