Breakthrough bleeding journey

I have been on HRT now for 3 years and have constantly had issues with breakthrough bleeding. I have had kliovance/kliofem but then realised after investigating you can have malabsorption problems after stomach surgery and  was advised to have patches. I was put on Evorel Conti but felt I needed higher estrogen as it didn’t control all my symptoms. I was then given Evorel Conti and an extra 50mg just estrogen Conti patch. Then about 6 months later I had a call from our pharmacist saying this was wrong as I wasn’t getting enough progesterone and was prescribed 2 Evorel Conti patches. I was then called back to GP for another consultation, I hadn’t  tried utrogestan because of potential absorption problems but after discussing GP was happy to prescribe me utrogestan but to be used vaginally. I was sent away with an evorel 100 patches  and to take 200mg utrogestan every day which I feel great on but my only side effects seem to be spotting less blood than before but still here.

**** Apologies TMI but I need to ask 🫤 **** I am now a month in to this regime and feel fine on it but still seem to have a lot of powdery discharge which I imagine is leftover bits of the utrogestan mixed with thin stringy brown bits. Not enough to wear a tampon or pad just a bit yucky. Is this the utrogestan thinning out stuff as I feel I may not of had sufficient progesterone cover previously? The bleeding is nothing like before where I was getting clots etc. But will the spotting stop as you’d imagine 200mg per day you would?

My journey has been very up and down but now have a GP who is very understanding and supportive and is happy to tweak things until we get it right as feels with my HRT journey stuff had been prescribed that wasn’t suitable for me. She has suggested if this doesn’t work to try the Mirena coil and would be happy to push for this for me as although it’s not the norm for our area to offer the Mirena for HRT (only birth control) given my history and problems I would get it as I’ve been sent to the bleed clinic for ultrasounds etc.

Sorry for the long post but I’m not sure what to do next as although I feel good on it the bleeding side is a nuisance or is this as good as it gets?

At least your GP is trying to find a regime that suits.

The powerdery discharge is probably the packaging from the Utrogestan, [the stringy 'bits' maybe old blood that hasn't come away due to a lack of 'push' from the hormones].

It seems strange that Mirena isn't given as part of HRT, R U in Scotland>

Let us know how you get on.  If you feel good and can be reassured about the bleeding, I would stick with it for at least 6 months.  The body doesn't get where it is suddenly and may take a while to uptake the HRT .

Many thanks CLKD, yes I am in Scotland but my lovely GP feels I could get it due to trying everything else. I’ve chopped and changed so much I want to give this a go so will persevere I am due to see her again at the 3 month stage if I’m still having breakthrough bleeds I’ll probably ask to go on the wait list for a Mirena coil as running out of options! Just wondered if it’s normal as assumed 200mg would just stop it but maybe I was being overly optimistic 😘

I was on evorel conti as well but had bleeding after the initial 6 months, dr then put me on utrogestan 100 x 2 oral tablets and evorel, all was well for 2 weeks no bleed whatsoever, but yep there's a but for me, I've started bleeding again over the weekend.  Hopefully your bleeding will stop. I will carry on taking mine but I'm ringing Dr up tomorrow for her to refer me for the mirena coil.  🤞 for you 😀

It's your own hormones joining in probably.  Let us know how you get on.

Thanks Pauline67 I’m the same started taking them and was totally bleed free for 10 days and then started spotting for over a week so far - much better then when I was on Evorel Conti but still there! I think the Mirena will be the way to go for me but thought I’d cracked it finally 😘😂
