Pelvic Pain - Menopause

Hi all, I'm new here and was really hoping to get some feedback from you please.

I've been experiencing really bad pelvic pain for the last 9 months...

 Was previously in the gym every day, weight training and cardio and very strong and fit. Initially I thought I had a back injury and that my hip/pelvic pain might be low back problem or Psoas muscle related. Lots of physio and scans later and the pelvic pain still persists. Minor back ache is ok, but the pelvic pain means that sitting is awful and everyday work at my desk is red it cing me to tears by the end of the day on many occasions.

I've gradually done less and less exercise as it's painful and feel like I've been on a downward spiral.

Finally I went back to my GP - ultrasound revealed I have a 5.6cm fibroid on the right hand side where I'm experiencing the pain. I've paid privately to see a gynecologist and she had our me on HRT and seems to think that the pain isn't necessarily related to the fibroid. I'm not sure if menopause could cause you'd story of pelvic pain and that HRT could help with this??

Have any of you had this type of pain that went away after starting hrt?

Where you mention having physio have you seen a specific women’s pelvic floor physio who will do an internal assessment of your pelvic floor ?

Hi!  Hormones may certainly affect fibroids.  What did the gynae suggest regarding your pelvic pain ?  Hopefully didn't wave you away with X, Y 'isn't related' conversation?

Hello... Thank you for replying :)

So I have experienced low back pain and right hand side pelvic pain.

I've had lower back x-rays, plus MRI and also right hand side hip x-ray and all are ok.

I'm aware fibroid pain can cause you'd type of pelvic pain but consultant isn't sure it is the cause. She recommended HRT.

So I'm wondering what else could be the cause of right hand side pelvic pain if it isn't a fibroid? 🤷🏻‍♀️ (which also happens to be on the right hand side of my pelvis)

Could pelvic floor cause this type of localised pain with back pain? (All right hand side?? Seems more likely the fibroid to me, but I'm open to hearing how HRT might solve this?! 🙈

Worse case scenario Pudendal neuralgia is typically one sided pelvic pain causing difficulty with sitting.  Weight training doesn’t help.

If you have not seen a pelvic physio I would start there to be assessed in a different way to a gynaecologist.

Hi, I hope you find some answers. This is what I'm going through but no fibroid. I've been like this for around 2 years and like yourself started with lower back pain. Ive got pelvic pain almost on a daily basis and when my period comes it gets a whole lot worse. I haven't got any answers although endometriosis has been mentioned but still don't know. It all started with peri menopause was fine before. I'm at a loss with it now

Womens health physiotherapy for advice. 

Slinks27 - what did your GP/Consultant suggest if 'endometriosis' was mentioned? MayB find an appropriate support group and ask the question?  Then let us know.  Some medics don't know how endo might affect women!

I should add I'm one year into menopause, last period over a year ago...

No, endometriosis  hasn't been mentioned. Just multi fibroid, largest 5.6cm rhs, and bulky uterus...

The  pelvic pain is present when standing too, not just sitting so not pedundal neuralgia.

This is a constant rhs- pelvic ache that gets 'louder' as the day goes on. Some days its quieter. But pretty much constant. When it's bad it can wake me up. (Apart from the fact I'm up to the loo 3-4 times every night anyway 🙄)

R U getting up to pass urine or several poos?  I wonder if this might be worth using vaginal atrophy treatment to see whether the sensation is eased?

Hi Claire, it was the practice nurse that suggested it could be possible which was about a month ago. She gave me progesterone only pill and said to try that but it just made me feel so sick and my moods were worse than they are already so currently back at square one again. Would be great if icould get another appointment with her as she was really good and understanding but chances of that are near on impossible at the mo. She did, at my last appointment, mention if things don't settle with pill sending me for MRI but yet to be able to get an appointment, meanwhile, the pain goes on. I can barely even work now, cant have sex because of pain after and struggling to even clean the house, its debilitating

Yes I get it on standing also and some days I can start the day almost pain free and then the pain creeps in. I find tens machine helpful sometimrs

That could be fibroid or Meno related for sure 🙈

@slinks27 I think the big difference here may be the fact that you still have your periods possibly. (Apart from no fibroids) I'm pretty sure my pain I fibroid related but trying to investigate other possibilities I haven't already been able to rule out...

It could be the fibroid which may be aggravated by hormonal upheaval.  I remember having a low down pain on sitting/standing : can't remember where about I was in peri but I was very aware.

Yeah you maybe right. My period are very erratic at the moment, never know where I am. My last cycle was 17 days and one before 20 days yet back end of last year I kept missing. All very strange.

Hi Claire. Fibroids can cause intense pelvic pain in some women which can become quite disabling. Have you been offered any treatment apart from HRT. Oestrogen can cause fibroids to grow so it's not always advised if you have fibroids.
