Newbie needs help
« on: February 10, 2023, 12:43:57 PM »

Hi
I hope someone can help me. I am new to all of this.
I am 50 years old and started on HRT in October 2022. I am on Evorel patches and Utrogestan
Within days I was fine. And felt like my old self. Then over Christmas I started having  night sweats and early January I started spotting. I called the doctors and they changed my patch from 50 to 75. The night sweats stopped and I started to have severe PMT symptoms. Low mood, anger, crying all over nothing. And generally when at work.
I called the doctors yesterday and they wanted me to be signed off with anxiety and depression. And prescribed me antidepressants. Which I don’t really want to take if I can help it. I feel that I am certainly not depressed.
I have been spotting/bleeding for a month now and feel it is like I have PMT.
The doctor said that the Utrogestan doesn’t affect moods etc. but I have read that it can.
I guess what I want to know is has anyone else had this problem?
Would I be best to try the Mirena Coil?
Any help is really appreciated.
Thanks in advance xx

Hello JoP and welcome. I am sorry you are feeling so low. Ultrogestan is very well known for causing low mood so your GP is incorrect. This forum has a lot of threads on ultrogestan making people severely depressed. How sad your GP is unaware of this. You can try the Mirena instead of the Ultrogestan. :)

Hi Flossieteacake
Thanks for your reply. Hmmm I thought that was the case. I will check the other threads on here too.
So glad it seems I am not going totally mad with this. Thank you x

You are not going mad. I think you are very aware as you realised you do not have depression but connected it to the ultrogestan.

Another GP who knows not a lot  >:(

:welcomemm:  we knows a lot more ;-)

Some find that keeping a mood/food/symptom diary of use.  Ring your surgery and ask for a referral to an NHS menopause clinic, don't be fobbed off with a gynaecologist!

Let us know how you get on.  There mayB a Nurse Practitioner with more knowledge about peri-menopause than the GP/s seem to.  You could check with the Practice Manager.

Do you take utrogeston every day (conti) or on a cycle (sequi)? Are you meno or peri? If you're on conti you could change to a sequi regime so you'll know if utrogeston is causing the problem.

I am peri. I take it for 25 days then have a break. I am also on the mini pill for contraception. I didn’t know whether to try the Mirena coil. I don’t have the symptoms every day. Yesterday and today I am perfectly fine.

You could look at the dose of progesterone in the mini pill as you may be taking too much. A mirena would solve the problem as it provides both contraception and the progesterone part of hrt. I think taking a contraceptive pill as well as hrt is often unsuccessful. A sequi regime on utro is 200mg x 12 days (days 15-26) but again alongside the mini pill is likely to be too much.

Thank you Sheila99 this is what I have been wondering. I think I will make an appointment for the mirena coil and hope that works for me.
Thank you for your help xx

What sheila said ^^^^^^ !!!

I would go for Mirena if you still need contraception and are on mini pill and utrogestan - and that will then cover both contraception and the progestogen part of HRT and protect your uterus. Which mini pill are you on?

Hi Hurdity

I’m on Desogestrel 75mg. I think I will try the Mirena.
The doctor signed me off with anxiety and depression and prescribed antidepressants but I know I am not depressed. I would rather try different things as I don’t really want to go down that route. And the doctors haven’t offered me any alternative.
I’ve been fine since Thursday and have carried on working. It comes and goes. I do have a high pressured job and some days just feel useless and tearful over nothing at all.

Thanks for your help xx
