Gynae appointment today

I’ve been referred to a gynae consultant this afternoon for abnormal bleeding. This will be the second time and the last consultant I saw last summer said he was satisfied everything was ok.
I had a second Mirena removed in October 2021 then had a bleed (normal I was told) then another out of the blue v normal period in May 2022. I had a number of ultrasounds and they found I had a v small fibroid but all ok. I then started with sore boobs and classic period in December and had another trans vaginal ultrasound. The lady doing it assured me all ok and womb lining was 3mm.
My Gp said that was on the cusp of needing to be investigated but everything I’ve read seems to say otherwise. I’m on continuous Utrogestan and two pumps of oestrogen and meno symptoms well controlled- lady doing ultrasound said I was perimeno and not postmeno at all as Gp seems to have been treating me.
Basically I don’t want to go through further tests if they’re not really needed! Any thoughts??

Why did the lady say you are in peri meno ? Not sure how she can tell that from a scan ? Can you tell us how old you are ? x

I’m 56. She said she could see it from my ovaries? I think she said cysts on them so still productive??

Hi the definition of menopause is 12 months without a period. I’ve never heard of what you’ve been told.

At the age of 56 you are likely to be in menopause but not definitely. Some ladies are still having regular periods at your age .

The BMS guidance is to double the Utrogestan if you’re still bleeding and see if that works. However if you’re in peri you’d be better taking the Utrogestan on a cyclical basis .

A lining of 3mm doesn’t usually mean further investigation. I think it’s usually 4mm but I’m not 100% sure on that.

I’d go ahead with the appt and see what they say. Sorry I’m not being much help. x

Yes I've never understood why I was told to take 100mg Utrogestan continuously.  My GP told me to take 200mg to see if that helped, but it just made me feel anxious and jittery.  She then started talking about how the Mirena is the gold standard blah blah - I've had two in and really don't particularly want a third at 56.
My periods were very heavy which is why I went onto it, I then didn't have any periods on it, which I expected.  When my second one came out, my GP decided to do blood tests to determine whether I was menopausal.  They decided on the basis of these bloods and my age that I was, and so any bleeds now are deemed to be odd....!
I had two periods this year, well spaced out, but they were as I would describe as 'normal' with PMS breast soreness etc., beforehand.
I'm going to go along today but armed with my history (the last consultant I had didn't have that at all!), and see what they think.  I'm trying to avoid going down the biopsy/hysteroscopy routes if necessary.  I've had them before with trying to conceive testing (blocked fallopian tubes and subsequent two rounds of IVF) and really am sick of being prodded around!

Oh dear, perhaps your gp has heard the fairy story that everyone is meno by 54, I have had similar problems because of it. If it feels like a period that's probably because it is one. Perhaps you're close enough to meno it doesn't matter but in peri imo you'd be better either on a sequi regime so you have a predictable bleed or using a mirena.
I contacted my gp aged 60 because hrt didn't induce a bleed every month and mentioned the 2 periods I'd had that year. Cue panic and investigation because of 'unexpected bleeding'. Everything normal and consultant said if I'd been younger they wouldn't have done anything. Some women aren't meno til their early 60s but many medics don't seem to realise.

I saw the consultant this afternoon. I’d gone very well prepared and written down my full history.
She said I was still having periods so not post meno and that as we know the blood tests are only a snapshot at any given time.
I’m going to give 200mg vaginal Utrogestan a go- was taking orally before. I’ve been discharged which is a huge relief!
